      目的:探讨老年人高血压前期糖耐量降低(IGT)对血管内皮功能和颈动脉粥样硬化的影响。方法:选择2013 年1 月至2014 年1 月在我院门诊、住院和体检中心纳入的60 岁以上志愿者240 例,依据血压、血糖水平将志愿者分为四组,单纯高血压前期 (PH)组,单纯IGT(IGT)组,高血压前期并发IGT(PH&IGT)组,正常对照(NC)组,每组均为60 例。检测颈动脉内中膜厚度(IMT) 和内皮依赖性血管舒张功能(FMD)并进行相关性分析。结果:与NC组相比,IMT 在PH组、IGT组和PH&IGT组显著升高[(0.74± 0.09)mm、(0.76± 0.12)mm、(0.96± 0.14)mmvs(0.57± 0.08)mm,均P<0.05],且PH&IGT组相比于PH组和IGT组进一步升高(P<0.05);与 NC 组相比,FMD 在PH组、IGT 组和PH&IGT 组显著降低[(8.77± 2.56) %、(8.59± 2.37) %、(5.78± 2.11)%vs(12.48± 2.78)%,均 P<0.05],且PH&IGT 组相比于PH组和IGT 组进一步下降(P<0.05)。直线相关分析显示,IMT 与FMD 呈显著负相关,相关性检验 有统计学意义(r=-0.843,P<0.01)。结论:高血压前期患者血管内皮功能受损,在高血压前期基础上的IGT 将会加剧血管内皮功能 障碍和动脉粥样硬化。

      Objective:To study the influence of impaired glucose tolerance(IGT) on endothelial function and carotid atherosclerosis in elderly patients with prehypertension.Methods:A total of 240 volunteers (age≥ 60 years old) in outpatient, inpatient and medical center of Qingdao Haici Medical Group from January 2013 to January 2014 were selected and divided into normal control group(NC group, n=60), prehypertension group(PH group, n=60), IGT group(n=60), prehypertension and IGT group(PH&IGT group, n=60) according to the blood pressure and blood glucose level. The intima-media thickness (IMT) and flow mediated diameter (FMD) in the four groups were detected, and their correlation was analyzed.Results:Compared with NC group, IMT in PH, IGT and PH&IGT groups significantly increased [(0.74± 0.09)mm, (0.76± 0.12)mm and (0.96± 0.14)mm vs (0.57± 0.08)mm, all P<0.05]; IMT in PH&IGT group significantly increased compared with PH and IGT group(P<0.05); FMD in PH, IGT and PH&IGT groups significantly decreased[(8.77± 2.56)%, (8.59± 2.37)%, and (5.78± 2.11)% vs (12.48± 2.78)%, all P<0.05], FMD in PH&IGT group significantly decreased compared with PH and IGT group (P<0.05). Linear correlation analysis showed that FMD was negatively correlated with IMT (r=-0.843, P<0.01).Conclusion:Vascular endothelial function is obviously impaired in elderly patients with prehypertension, on the basis of which, IGT will exacerbate the endothelial dysfunction and atherosclerosis.
