      目的:比较应用alpha- 硫辛酸抗氧化应激治疗联合胰岛素强化降糖对初诊2 型糖尿病患者胰岛beta细胞功能的影响。方法:将 104 例初诊2 型糖尿病患者随机分为联合组、单药组两组,分别给予alpha- 硫辛酸联合胰岛素强化治疗及单独胰岛素强化治疗12 周,比较两组患者治疗前后口服葡萄糖耐量试验(OGTT)、C 肽释放试验各时间点血浆葡萄糖、C 肽及胰岛功能相关指标的变化。 结果:联合组OGTT 试验1 小时血糖(1 h PG)、2 小时血糖(2 h PG)、3 小时血糖(3 h PG)较单药组明显下降,空腹C 肽(FCP)、0.5 小时C 肽(0.5 h CP)、2 小时C 肽(2 h CP)水平较单药组明显升高,胰岛茁细胞功能指数[HOMA-beta(CP)]、糖负荷0.5 h时净增C 肽与净增血糖比值(△CP30/△Glu30)、C 肽曲线下面积(AUCC)与血糖曲线下面积(AUCG)比值(AUCG/AUCC)较单药组明显升 高,差异均有统计学意义(P<0.05)。结论:alpha- 硫辛酸联合胰岛素强化治疗较单独胰岛素强化治疗能够更好的恢复胰岛茁细胞功能。

      Objective:To evaluate the effect of alpha-lipoic acid in combination with intensive insulin therapy on the recovery and maintenance of beta-cell function in newly diagnosed type 2 diabetic patients.Methods:104 newly diagnosed type 2 diabetic patients (T2DM) were randomly divided into two groups: Combined Group and Insulin Alone Group. The two groups were respectively treated with alpha-lipoic acid combined with intense insulin therapy and intense insulin alone for 12 weeks. The oral glucose tolerance test (OGTT) and C-peptide released test were performed before and after therapy, the plasma glucose and C-peptide at various points, and beta- cell function related indexes were compared.Results:After 12 weeks therapy, the 1-hour plasma glucose (1 hPG), 2-hour plasma glucose( 2 hPG) and 3-hour plasma glucose(3 hPG) in Combined Group were significantly decreased than those in Insulin alone group, while the fasting plasma C-peptide (FCP), the 0.5-hour plasma C-peptide (0.5 hCP) and 2-hour plasma C-peptide (2 hCP) in Combined Group were significantly increased than those in Insulin alone group. The beta-cell function of homeostasis model assessment [HOMA-beta(CP)], ratio of the first 30 minutes increase in C-peptide to glucose concentration(△CP30/△Glu30) and ratio of area under the curve(AUC) of C-peptide (AUCC) to AUC following OGTT(AUCC/AUCG) in Combined Group were significantly increased than those in Insulin alone group. Differences are statistically significant(P<0.05).Conclusion:The results suggest that LA combined with intensive insulin therapy can better recover the islet function than only intensive insulin therapy in newly diagnosed type 2 diabetes.
