      目的:探讨大直径陶瓷- 陶瓷假体对髋关节置换患者术后假体磨损,稳定性和髋关节功能的影响。方法:选取我院2010 年1 月-2014 年1 月间采用大直径陶瓷- 陶瓷假体行关全髋置换的股骨头坏死患者46 例作为研究组。另选40 例采用标准金属- 聚 乙烯假体手术患者作为对照组。观察并比较两组患者假体磨损情况、髋关节稳定性和功能评分。结果:研究组患者术后无一例脱 位,发生假体松动1 例,Harris 评分为(82.04± 1.92)分;对照组术后发生假体脱位4 例,松动4 例,Harris 评分为(81.37± 1.27)分; 研究组假体稳定性和磨损程度优于对照组,差异具有统计学意义(P<0.05),但两组髋关节功能评分无显著差异(P>0.05)。发生假 体松动的患者术后血沉、IL-1 和IL-6 浓度均高于正常值,差异具有统计学意义(P<0.05)。结论:与标准假体相比,大直径陶瓷- 陶 瓷假体稳定性好,磨损率低,是全髋置换手术的良好假体。

      Objective:To investigate the effects of large diameter ceramic prosthesis on the stability and function of hip joint in total hip arthroplasty in patients with osteonecrosis.Methods:46 cases with large diameter ceramic ceramic prosthesis in hip replacement patients with femoral head necrosis were selected in our hospital from January 2010 to 2014, and another 40 cases with standard prosthesis operation were selected as the control group. Then the stability and functional difference of the hip prosthesis between the two groups were compared.Results:The X-ray showed 1 cases of loosening of the prosthesis, and the score of postoperative Harris hip was (82.04± 1.92). For the standard prosthesis replacement group, there was 4 cases of dislocation after two years, 2 cases of loosening of the prosthesis, and Harris score of (81.37± 1.27). There was statistical significance in the stability and prosthetic wear degree between the two groups (P<0.05), but no significant differences in hip function (P>0.05). IL-1 and IL-6 concentration of patients with prosthetic loosening were significantly higher than those of the control group, and the blood sedimentation and CRP were higher than those of the control group with statistically significant differences (P<0.05).Conclusion:Compared with standard prosthesis, large diameter ceramic prosthesis has good stability, low wear rate, and can be used as total hip replacement.
