目的:通过检测幼年特发性关节炎(JIA)患者血清中的抗RA33 抗体,了解抗RA33 抗体与幼年特发性关节炎的临床诊断价
值。方法:采用酶联免疫固相分析检测81 例JIA 患儿(女19 名,男62 名,平均年龄8.6 岁,平均病程1.4 年)血清中抗RA33 抗
体、RF,同时以55 例儿童系统性红斑狼疮(SLE)等其他关节性疾病或病毒感染患者和49 例健康儿童作为对照组。阴阳性结果判
断均采用试剂盒推荐的临界值。结果:81 例JIA 患儿中抗RA33 抗体阳性率为11.11%(9/81),RF 阳性率为12.35%(10/81), 特异性
均为91.35%;JIA 组与正常对照组抗RA33 抗体阳性率比较有统计学意义(P<0.05),与其他关节性疾病对照组比较差异无显著性
(P>0.05)。JIA 组中抗RA33 抗体的检出与RF 无相关性(P>0.05);在JIA 各亚型中抗RA33 抗体主要存在于全身型和多关节型,各
占33.3%和25.0%,RF 则只出现于多关节型,占62.5%。两者比较有显著性差异(P<0.05)。81 例JIA 患儿中共有18 例关节出现影
像学改变,其中4 例抗RA33 抗体阳性(22.2%),与未发生影像学改变的JIA 患儿比较无显著性差异(P>0.05)。结论:抗RA33 抗体
尚不能作为JIA 早期诊断的新的可靠性指标,抗RA33 抗体主要见于全身型和多关节型,对JIA 的分型有指导意义。 |

Objective: To evaluate the clinical diagnostic value of anti-RA33 in patients with Juvenile idiopathic arthritis(JIA).
Methods: The level of serum Anti-RA33 antibody and rheumatoid factor (RF) were detected by Enzyme linked immunoabsorbent assay
(ELISA) in serum samples from 81 cases with JIA (including 19 female,62 male,with average age of 8.6 years,with an average history of
1.4 years) , while 55 cases with SLE,other joint diseases or virus infections and 49 normal children were enrolled to serve as control. The
positive and negative results are diagnosed by critical value of kit. Results: The positive ratio of anti-RA33 and RF in 81 JIA cases were
11.11%(9/81) and 12.35%(10/81),respectively ,compared with nomal control, has statistical significance (P<0.05).But the positive ratio
of anti-RA33 and RF has no significant different between the JIA group and other joint disease group (P>0.05).There is no dependability
between RF and the detection of anti-RA33 in JIA group(P>0.05).In all kinds of JIA hypotype, anti-RA33 antibody is principally reside in
systerm arthritis and polyarthritis,respectively, 33.3% and 25.0% ,which has significantly difference (P<0.05) .RF only appears in
polyarthritis, with 62.5 percent. 18 cases have imaging changes in those 81 JIA cases and 4 cases of which has positive anti-RA33
antibody(22.2%).Compared to JIA chidren without imaging changes ,there is no significant difference(P>0.05). Conclusion: Anti-RA33
antibody still can not be recognized as new and reliability index to diagnostic efficiency in children with early JIA.Anti-RA33 are
principally reside in systerm arthritis and polyarthritis,which may provide guidance on the classification of JIA. |
