      目的:通过定量CT测量黑龙江地区人群腹部脂肪面积,来探究其与血脂、血糖的关系。方法:选取于哈医大附属一院体检中心行肺部CT检查、20周岁以上的患者411例,年龄范围23~90岁,中位年龄58岁。分别进行体格检查和生化指标测定,根据体重指数(Body Mass Index, BMI)值把患者划分为正常组、超重组及肥胖组,并使用定量CT(quantitative CT ,QCT)脂肪测量软件定量测量患者L2~3水平层面腹部内脏脂肪面积(visceral fat area,VFA)和皮下脂肪面积(subcutaneous fat area,SFA)。将VFA和SFA数据分组, 用K-W检验检测各组间VFA值、SFA值的差异,并对各组间定量资料进行相关性分析。结果:三组间VFA值(P=0.000、0.000、0.033)及SFA值(P=0.000、0.000、0.000)均有显著性差异 (P均<0.05),VFA值在三组间均与血脂、血糖有显著的相关性。绘制VFA诊断甘油三酯(TG)及空腹血糖(FBG)值正常与否的ROC曲线图,当VFA分别为166.035cm2和172.65cm2时,对于诊断TG、FBG异常有着最高的敏感度及特异度。结论:BMI正常组及非正常组人群腹部脂肪面积具有显著差异, 且VFA的升高会增加高血脂、高血糖的患病风险。VFA对于评估TG、FBG值是否正常具有重要价值。

      ABSTRACT Objective:Through quantitative CT measurement of abdominal fat in Heilongjiang area, to explore the relationship between the area of abdominal fat and blood lipid, blood glucose. Methods:A total of 411 patients with the CT examination of the lungs who were older than 20years old were enrolled in the physical examination center of the First Affiliated Hospital of Harbin Medical University.The age ranged from 23 to 90 years old and the median age was 58 years. Physical examination and biochemical indicators were performed separately. According to the Guidelines for Prevention and Control of Adults Overweight and Obesity in China, patients were divided into normal group, super-recombination and obesity group according to BMI, and quantitative CT (quantitative CT, QCT) fat measurement was used. The software quantitatively measures the visceral fat area (VFA) and subcutaneous fat area (SFA) of the patient''s L2~3 level. The VFA and SFA data were grouped, and the difference of VFA value and SFA value between each group was detected by K-W test, and the correlation data between the groups were analyzed. Results:There were significant differences in VFA and SFA between the three groups (p<0.05). The VFA values were significantly correlated with blood lipids and blood glucose in the three groups. The ROC curve of VFA diagnosis of normal triglyceride (TG) and fasting blood glucose (FBG) values was obtained. When VFA was 166.035 cm2 and 172.65 cm2, respectively, the highest sensitivity and specificity were found for the diagnosis of TG and FBG abnormalities. Conclusions:There is a significant difference in abdominal fat area between the normal and non-normal BMI groups, and the increase in VFA may increase the risk of hyperlipidemia and hyperglycemia. VFA is of great value for evaluating whether TG and FBG values are normal.
