| 摘要 目的:本研究旨在对比分析原位自体牙冠即刻修复与CAD/CAM切削冠传统即刻修复在前牙即刻种植修复中的效果。方法:选取2023年8月至2024年1月于医院口腔种植综合科就诊的40例前牙单牙缺失患者,分为研究组和对照组,每组20例。研究组行自体牙冠即刻修复,对照组进行CAD/CAM切削冠传统即刻修复,均采用Straumann SLA BLT种植体,并于即刻修复当天、术后12个月复查。结果:两组在戴牙后即刻及种植术后12个月的种植修复成功率均为100%,差异无统计学意义。在美学评分上,研究组在戴牙后即刻及术后12个月的白色美学评分(WES)均高于对照组(P<0.05),而红色美学评分(PES)两组间差异无统计学意义。种植体周围骨水平评估中,两组BIC差异无统计学意义,但术后12个月种植体周围骨水平均高于戴牙即刻(P<0.05)。种植体周围牙周组织评估显示,戴牙即刻与术后12个月时,研究组的龈乳头指数(PI)高于对照组,出血指数(BI)低于对照组(P<0.05),探针深度(PD)两组差异无统计学意义。结论:原位自体牙冠即刻修复在前牙即刻种植修复种植修复成功率与CAD/CAM切削冠传统即刻修复相比无差异,但在美学效果及部分牙周组织健康指标上具有一定优势,为临床提供了一种有效的治疗选择。 |

| ABSTRACT Objective: This study aims to comparatively analyze the immediate restoration of in-situ autogenous crowns and CAD/CAM cutting. The effect of traditional immediate restoration of crowns in immediate implant restoration of anterior teeth. Methods: 40 patients with single missing anterior teeth who visited the Department of Oral Implantology of Hospital from August 2023 to January 2024 were selected and divided into a research group and a control group, with 20 cases in each group. The study group underwent immediate autologous crown repair, and the control group underwent traditional CADs / CAM cutting crowns, both with Straumann SLA BLT implants and reviewed on the day of immediate repair and 12 months after surgery. Results: The success rate of implant restoration in both groups immediately after tooth placement and 12 months after implantation was 100%, and the difference was not statistically significant. In terms of aesthetic scores, the white aesthetic score (WES) of the study group was higher than that of the control group immediately after wearing the teeth and 12 months after surgery (P<0.05), while there was no statistical difference in the red aesthetic score (PES) between the two groups. In the assessment of bone levels around implants, there was no statistically significant difference in BIC between the two groups, but the bone levels around implants 12 months after surgery were higher than immediately after tooth placement (P<0.05). Evaluation of the periodontal tissue around the implant showed that immediately after wearing the teeth and 12 months after the operation, the gingival papilla index (PI) of the study group was higher than that of the control group, and the bleeding index (BI) was lower than that of the control group (P<0.05). There was no statistically significant difference in needle depth (PD) between the two groups. Conclusion: The success rate of immediate implant restoration in anterior tooth is not different compared with CAD / CAM cutting crown traditional immediate restoration, but it has some advantages in aesthetic effect and some periodontal tissue health indicators, which provides an effective treatment option for clinical practice. |
