      摘要 目的:探讨血管性痴呆患者执行、记忆、认知功能与脑电图的关系。方法:选择2021年5月至2023年5月来我院诊治的非痴呆型血管性认知障碍患者40例作为A组、血管性痴呆患者35例作为B组,选择健康志愿者30例作为C组。所有患者使用数字化脑电监护仪进行定量脑电图检查,进行额叶功能评定量表检查、执行性画钟作业(CDT)、Stroop测验、词语流畅性测验、记忆力及认知功能检测。对比三组受试者每个频带的绝对功率值、(δ+θ)/(α+β)值,分析三组受试者的额叶功能评分、CDT评分、Stroop测验评分、词语流畅性测验评分,分析三组受试者的记忆力评分与认知功能评分,分析B组患者的定量脑电图检查项目与各项评分的相关性。结果:A、B组受试者的颞叶、额叶、枕叶、顶叶、中央的(δ+θ)/(α+β)值明显较C组高,B组以上指标明显较A组高(P<0.05)。A、B组的额叶功能评分、CDT评分、Stroop2、词语流畅性测验评分明显较C组低,Stroop1明显较C组高;B组的额叶功能评分、CDT评分、Stroop2、词语流畅性测验评分明显较A组低,Stroop1明显较A组高(P<0.05)。A、B组的瞬时记忆、短延迟记忆、长延迟记忆、再认记忆、认知功能评分明显较C组低,B组以上评分明显较A组低(P<0.05)。血管性痴呆患者的颞叶、额叶、枕叶、顶叶、中央脑电图与额叶功能评分、CDT评分、Stroop2、词语流畅性测验评分、瞬时记忆、短延迟记忆、长延迟记忆、再认记忆、认知功能评分呈负相关,与Stroop1呈正相关(P<0.05)。结论:血管性痴呆患者存在明显的执行、记忆、认知功能障碍,脑电图检测可评价其严重程度。

      ABSTRACT Objective: To investigate the relationship between executive, memory, cognitive function and electroencephalogram (EEG) in patients with vascular dementia. Methods: Select 40 non dementia vascular cognitive impairment patients who came to our hospital for treatment from May 2021 to May 2023 as Group A, 35 vascular dementia patients as Group B, and 30 healthy volunteers as Group C. All patients underwent quantitative electroencephalogram (EEG) examination using digital EEG monitor, frontal lobe Function Rating Scale (FSMA) examination, CDT, Stroop test, word fluency test, memory and cognitive function examination. The absolute power and (δ+θ)/(α+β) values of each frequency band of the three groups were compared, and the scores of frontal lobe function, CDT, Stroop test score and word fluency test score of the three groups were analyzed, and the scores of memory and cognitive function of the three groups were analyzed. The correlation between the quantitative EEG examination items and each score in group B was analyzed. Results: In the subjects in group A and B, the (δ+θ)/(α+β) values in the temporal lobe, frontal lobe, occipital lobe, parietal lobe and central lobe were higher than those in group C, and the above indexes in group B were significantly higher than those in group A (P<0.05). The scores of frontal lobe function, CDT score, Stroop2 and word fluency test in group A and B were lower than those in group C, and Stroop1 was higher than those in group C. The scores of frontal lobe function, CDT score, Stroop2 and word fluency test in group B were lower than those in group A, and Stroop1 was significantly higher than those in group A (P<0.05). The scores of instantaneous memory, short delayed memory, long delayed memory, recognition memory and cognitive function in groups A and B were significantly lower than those in group C, and the above scores in group B were lower than those in group A (P<0.05). Temporal, frontal, occipital, parietal, and central EEG were negatively correlated with frontal function score, CDT score, Stroop2, word fluency test score, instantaneous memory, short delay memory, long delay memory, rerecognition memory, and cognitive function score, and positively correlated with Stroop1 (P<0.05). Conclusion: Vascular dementia patients have obvious executive, memory, cognitive dysfunction, EEG detection can evaluate its severity.
