摘要 目的:研究慢性阻塞性肺疾病(COPD)患者血清25-羟基维生素D[25(OH)D]、单核细胞趋化蛋白1(MCP-1)、五聚素3(PTX3)水平与骨质疏松症的关系。方法:选择2021年12月至2023年11月期间我院收治的COPD患者182例(COPD组)和同期来我院体检的95例体检健康者(对照组)。根据是否发生骨质疏松症将COPD患者分为骨质疏松症组和非骨质疏松症组。收集COPD患者一般资料,COPD患者并发骨质疏松症的影响因素采用单因素和多因素Logistic回归分析。结果:对照组、非骨质疏松症组、骨质疏松症组血清25(OH)D水平依次下降,血清MCP-1、PTX3水平依次升高(P<0.05)。多因素Logistic回归分析结果显示,高龄、有糖皮质激素使用史、低体质量指数、有吸烟史、血清25(OH)D水平下降、血清MCP-1、PTX3水平升高是COPD患者并发骨质疏松症的危险因素(P<0.05)。结论:血清25(OH)D水平下降与血清MCP-1、PTX3水平升高共同促进COPD并发骨质疏松症患者病情进展。 |

ABSTRACT Objective: To investigate the relationship between serum 25-hydroxyvitamin D [25 (OH) D], monocyte chemoattractant protein 1 (MCP-1), pentraxin 3 (PTX3) levels in patients with chronic obstructive pulmonary disease (COPD) and osteoporosis. Methods: 182 COPD patients (COPD group) admitted to our hospital from December 2021 to November 2023 and 95 healthy individuals (control group) who underwent physical examination in our hospital during the same period were selected. COPD patients were divided into osteoporosis group and non- osteoporosis group based on the occurrence of osteoporosis. General information of COPD patients was collected, and the influencing factors of osteoporosis in patients with COPD were analyzed using univariate and multivariate Logistic regression analysis. Results: Serum 25 (OH) D levels in control group, non-osteoporosis group and osteoporosis group were decreased successively, while serum MCP-1 and PTX3 levels were increased successively(P<0.05). Multivariate Logistic regression analysis showed that advanced age, history of glucocorticoid use, low body mass index, smoking history, decreased serum level of 25(OH)D, and increased serum levels of MCP-1 and PTX3 are risk factors for osteoporosis in patients with COPD (P<0.05). Conclusion: Decreased serum level of 25(OH)D and increased serum levels of MCP-1 and PTX3 jointly promote the progression of osteoporosis in patients with COPD. |
