摘要 目的:观察早期肺康复训练与全身振动训练联合治疗对急性加重慢性阻塞性肺疾病(AECOPD)患者肺功能、运动能力和动脉血气分析指标的影响。方法:根据随机数字表法将我院136例AECOPD患者分为对照组(早期肺康复训练,n=68)和观察组(对照组的基础上接受全身振动训练,n=68)。对比两组慢性阻塞性肺疾病评估测试(CAT)、肺功能指标、圣乔治呼吸问卷(SGRQ)评分、动脉血气分析指标、运动能力、6个月内急性加重次数、30 d再入院率。结果:观察组治疗后CAT、SGRQ评分、二氧化碳分压(PCO2)低于对照组,第1秒用力呼气容积(FEV1)、用力肺活量(FVC)、呼气峰值流速(PEF)、动脉氧分压(PO2)、血氧饱和度(SPO2)、六分钟步行距离(6MWD)、30 s椅子起坐试验(30-STS)高于对照组(P<0.05)。观察组的30 d再入院率及6个月内急性加重次数均少于对照组(P<0.05)。结论:AECOPD患者采用早期肺康复训练联合全身振动训练,可改善其肺功能、运动能力和动脉血气分析指标,同时还可降低6个月内急性加重次数以及30 d再入院率。 |

ABSTRACT Objective: To observe the effects of combine treatment of early pulmonary rehabilitation training and whole body vibration training on pulmonary function, exercise capacity and arterial blood gas analysis index in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). Methods: 136 AECOPD patients in our hospital were divided into control group (early pulmonary rehabilitation training, n=68) and observation group (whole body vibration training on the basis of control group, n=68) according to the random number table method. The chronic obstructive pulmonary disease assessment test (CAT), pulmonary function index, St. George 's respiratory questionnaire (SGRQ) score, arterial blood gas analysis index, exercise capacity, number of acute exacerbations within 6 months, 30-day readmission rate were compared between two groups. Results: The CAT, SGRQ scores and partial pressure of carbon dioxide (PCO2) in observation group were lower than those in control group after treatment, and the forced expiratory volume in the first second (FEV1), forced vital capacity (FVC), peak expiratory flow (PEF), arterial oxygen partial pressure (PO2), oxygen saturation (SPO2), six-minute walking distance (6MWD) and 30 s chair sit-up test (30-STS) were higher than those in control group (P<0.05). The 30-day readmission rate and the number of acute exacerbations within 6 months in observation group were less than those in control group(P<0.05). Conclusion: Early pulmonary rehabilitation training combined with whole-body vibration training can improve pulmonary function, exercise capacity and arterial blood gas analysis indexes in AECOPD patients, while also reducing the number of acute exacerbations within 6 months and the 30-d readmission rate. |
