      摘要 目的:探讨右美托咪定(Dex)联合肺表面活性物质(PS)对脂多糖诱导的急性呼吸窘迫综合征(ARDS)大鼠肺损伤、血流动力学的影响。方法:将50只SD大鼠随机分为Control组、Model组、Dex组、PS组、Dex+PS组。Control组注射生理盐水,其余组建立大鼠 ARDS 模型,Dex组、PS组、Dex+PS组分别进行Dex、PS、Dex联合PS给药,其余组灌注生理盐水。多普勒超声仪检测大鼠血流动力学变化;检测大鼠肺功能;血气分析仪检测氧分压(PaO2)和二氧化碳分压(PaCO2)并计算氧合指数(OI);酶联免疫吸附法检测肺泡灌洗液肿瘤坏死因子-α(TNF-α)、白介素(IL)-6、IL-1β水平。结果:与Control组相比,Model组大鼠最大呼气流量(PEF)、用力肺活量(FVC)、0.3秒的用力呼气量(FEV0.3)/FVC水平、左心室收缩压(LVSP)、心率(HR)、平均动脉压(MAP)、PaO2、OI水平降低,PaCO2水平、肺泡灌洗液中TNF-α、IL-6、IL-1β水平升高(P<0.05)。与Model组相比,Dex组、PS组和Dex+PS组PEF、FVC、FEV0.3/FVC水平、LVSP、MAP、HR水平、PaO2、OI水平升高,且Dex+PS组高于Dex组和PS组(P<0.05);PaCO2水平、肺泡灌洗液中TNF-α、IL-6、IL-1β水平降低,且Dex+PS组低于Dex组和PS组(P<0.05)。结论:Dex和PS联合治疗可减轻ARDS大鼠炎症和肺组织损伤,改善血流动力学和肺功能。

      ABSTRACT Objective: To investigate the effects of dexmedetomidine (Dex) combined with pulmonary surfactant (PS) on lung injury and hemodynamics in rats with lipopolysaccharide induced acute respiratory distress syndrome (ARDS). Methods: 50 SD rats were randomly divided into Control group, Model group, Dex group, PS group and Dex+PS group. The rats in control group were injected with normal saline, and the rats in other groups were established with ARDS model, Dex group, PS group and Dex+PS group were given Dex, PS and Dex combined with PS respectively, and the rats in other groups were perfused with normal saline. The hemodynamic changes in rats were detected by Doppler ultrasound. Lung function in rats was detected. Oxygen partial pressure (PaO2), carbon dioxide partial pressure (PaCO2) were detected by blood gas analyzer and the oxygenation index (OI) was calculated. The levels of tumor necrosis factor-α (TNF-α), interleukin (IL)-6 and IL-1β in bronchoalveolar lavage fluid were detected by enzyme-linked immunosorbent assay. Results: Compared with Control group, peak expiratory flow (PEF), forced vital capacity (FVC), forceful expiratory volume in 0.3 seconds (FEV0.3)/FVC levels, left ventricular systolic pressure (LVSP), heart rate (HR), mean arterial pressure (MAP), PaO2 and OI levels decreased in Model group. And PaCO2 level, TNF-α, IL-6 and IL-1β levels in alveolar lavage fluid increased(P<0.05). Compared with Model group, PEF, FVC, FEV0.3/FVC levels, LVSP, MAP, HR, PaO2 and OI levels were elevated in Dex, PS, and Dex+PS groups, and Dex+PS group were higher than those of Dex and PS groups(P<0.05). PaCO2, TNF-α, IL-6 and IL-1β levels in alveolar lavage fluid were decreased, and Dex+PS group was lower than that of Dex and PS groups(P<0.05). Conclusion: The combined treatment of Dex and PS can alleviate inflammation and lung tissue injury, improve hemodynamics and lung function in ARDS rats.
