      摘要 目的:探讨变应性鼻炎(AR)患者血清核苷酸结合寡聚化结构域样受体蛋白3(NLRP3)炎症小体及其下游炎症因子水平表达及其临床意义。方法:选取2021年4月至2023年9月我院收治的206例AR患者(AR组),根据病情严重程度分为轻度组(107例)和中重度组(99例),另选取我院同期体检健康的168例志愿者(对照组)。检测两组血清NLRP3炎症小体、白细胞介素(IL)-1β、IL-18水平。采用过敏性鼻炎评分量表(SFAR)评估AR患者的症状。Pearson法分析AR患者血清NLRP3炎症小体、IL-1β、IL-18与SFAR评分的相关性。受试者工作特征(ROC)曲线分析NLRP3炎症小体、IL-1β、IL-18诊断AR以及预测中重度AR的价值。结果:AR组血清NLRP3炎症小体、IL-1β、IL-18水平均高于对照组(P<0.05)。中重度组血清NLRP3炎症小体、IL-1β、IL-18水平、SFAR评分均高于轻度组(P<0.05)。AR患者血清NLRP3炎症小体、IL-1β、IL-18水平与SFAR评分均呈正相关(P<0.05)。NLRP3炎症小体、IL-1β、IL-18诊断AR的曲线下面积(AUC)为0.796、0.821、0.800,三指标联合诊断的AUC为0.930,高于各指标单独诊断。NLRP3炎症小体、IL-1β、IL-18预测中重度AR的AUC为0.793、0.770、0.736,三指标联合预测的AUC为0.893,高于各指标单独预测。结论:AR患者血清NLRP3炎症小体、IL-1β、IL-18均显著升高,且与AR病情加重有关。NLRP3炎症小体及其下游炎症因子联合检测对AR诊断和预测中重度AR具有较高价值。

      ABSTRACT Objective: To investigate the expression and clinical significance of serum nucleotide-binding oligomerization domain-like receptor protein 3(NLRP3) inflammasome and its downstream inflammatory factors in patients with allergic rhinitis (AR). Methods: 206 AR patients(AR group) admitted to our hospital from April 2021 to September 2023 were selected, and patients were divided into mild group (107 cases) and moderate to severe group (99 cases) according to the severity of the disease, another 168 healthy volunteers (control group) who underwent physical examination in our hospital during the same period were selected. The levels of serum NLRP3 inflammasome, interleukin (IL)-1β and IL-18 were detected in two groups. The symptoms of AR patients were evaluated by allergic rhinitis rating scale (SFAR). The correlation between serum NLRP3 inflammasome, IL-1β, IL-18 and SFAR score in AR patients was analyzed by Pearson method. The value of NLRP3 inflammasome, IL-1β and IL-18 in the diagnosis of AR and the prediction of moderate to severe AR were analyzed by receiver operating characteristic(ROC) curve. Results: The levels of serum NLRP3 inflammasome, IL-1β and IL-18 in AR group were higher than those in control group(P<0.05). The levels of serum NLRP3 inflammasome, IL-1β, IL-18 and SFAR score in moderate and severe group were higher than those in mild group (P<0.05). The levels of serum NLRP3 inflammasome, IL-1β and IL-18 in AR patients were positively correlated with SFAR score(P<0.05). The area under the curve (AUC) of NLRP3 inflammasome, IL-1β, and IL-18 in the diagnosis of AR was 0.796, 0.821, and 0.800 respectively, the AUC of the combined diagnosis of the three indicators was 0.930, which was higher than that of each indicator alone. The AUC of NLRP3 inflammasome, IL-1β and IL-18 in predicting moderate to severe AR was 0.793, 0.770 and 0.736 respectively, the AUC of the combined prediction of the three indicators was 0.893, which was higher than that of each indicator alone. Conclusion: The serum levels of NLRP3 inflammasome, IL-1β and IL-18 in AR patients are significantly increase, which are relate to the aggravation of AR. The combine detection of NLRP3 inflammasome and its downstream inflammatory factors has a high value in the diagnosis of AR and the prediction of moderate to severe AR.
