      摘要 目的:探讨血清淀粉样蛋白A(SAA)、白细胞介素-17(IL-17)、脂质运载蛋白2(LCN2)水平与慢性自发性荨麻疹(CSU)患者病情严重程度的关系。方法:选取2021年1月~2023年12月我院收治的CSU患者152例(CSU组)和同时间段130例体检健康者(对照组)。采用酶联免疫吸附法检测并比较CSU组和对照组血清SAA、IL-17、LCN2水平。根据7日荨麻疹活动度评分(UAS7)将CSU患者分为轻度CSU组(40例)、中度CSU组(65例)、重度CSU组(47例),比较三组血清SAA、IL-17、LCN2水平。Spearman法分析血清SAA、IL-17、LCN2水平与CSU患者UAS7的相关性,受试者工作特征(ROC)曲线分析血清SAA、IL-17、LCN2水平对CSU患者病情严重程度的预测价值。结果:与对照组比较,CSU组血清SAA、IL-17、LCN2水平升高(P<0.05)。轻度CSU组、中度CSU组、重度CSU组血清SAA、IL-17、LCN2水平依次升高(P<0.05)。血清SAA、IL-17、LCN2水平与CSU患者UAS7呈正相关(r s=0.537、0.616、0.573,P均<0.001)。血清SAA、IL-17、LCN2水平联合检测预测中度CSU与重度CSU的曲线下面积(AUC)分别为0.914、0.911,分别大于血清SAA、IL-17、LCN2水平单独预测的0.769、0.789、0.777与0.770、0.807、0.779(P<0.05)。结论:血清SAA、IL-17、LCN2水平升高与CSU患者病情严重程度有关,血清SAA、IL-17、LCN2水平联合检测预测CSU患者病情严重程度的效能较高。

      ABSTRACT Objective: To investigate the relationship between serum amyloid A (SAA), interleukin-17 (IL-17), lipid carrier protein 2 (LCN2) levels and the severity of disease in patients with chronic spontaneous urticaria (CSU). Methods: 152 CSU patients (CSU group) and 130 healthy subjects (control group) admitted to our hospital from January 2021 to December 2023 were selected. The levels of serum SAA, IL-17 and LCN2 in CSU group and control group were detected and compared by enzyme-linked immunosorbent assay. CSU patients were divided into mild CSU group (40 cases), moderate CSU group (65 cases) and severe CSU group (47 cases) according to the 7-day urticaria activity score (UAS7), the levels of serum SAA, IL-17 and LCN2 were compared in three groups. The correlation between serum SAA, IL-17, LCN2 levels and UAS7 in CSU patients was analyzed by Spearman method, the predictive value of serum SAA, IL-17, LCN2 levels on the severity of disease in CSU patients was analyzed by receiver operating characteristic (ROC) curve. Results: Compared with control group, the levels of serum SAA, IL-17 and LCN2 in CSU group were increased (P<0.05). The levels of serum SAA, IL-17 and LCN2 in mild CSU group, moderate CSU group and severe CSU group increased in turn (P<0.05). Serum SAA, IL-17 and LCN2 levels were positively correlated with UAS7 in CSU patients (r s=0.537, 0.616, 0.573, all P<0.001). The area under the curve (AUC) of combined detection of serum SAA, IL-17 and LCN2 levels in predicting moderate CSU and severe CSU were 0.914 and 0.911 respectively, which were greater than 0.769, 0.789, 0.777 and 0.770, 0.807, 0.779 predicted by serum SAA, IL-17 and LCN2 levels alone (P<0.05). Conclusion: The increase of serum SAA, IL-17 and LCN2 levels are relate to the severity of disease in CSU patients, the combine detection of serum SAA, IL-17 and LCN2 levels is more effective in predicting the severity of disease in CSU patients.
