摘要 目的:分析动脉瘤性蛛网膜下腔出血(aSAH)患者血清小窝蛋白-1(Cav-1)、血栓素A2(TXA2)的表达意义及对预后的评估效能。方法:选择我院自2021年1月至2023年1月收治的110例aSAH患者作为观察组,110例健康体检者作为对照组。检测所有受试者血清Cav-1、TXA2的表达水平,使用Spearman相关分析aSAH患者血清Cav-1、TXA2表达水平与WFNS分级、Hunt-Hess分级、改良Fisher分级的相关性,随访记录aSAH患者的预后情况,分析aSAH患者预后的影响因素,通过AUC评价血清Cav-1联合TXA2对aSAH患者预后的预测效能。结果:观察组血清Cav-1表达水平较对照组低,TXA2较对照组高(P<0.05);经Spearman相关分析,aSAH患者WFNS分级、Hunt-Hess分级、改良Fisher分级均与血清Cav-1表达水平呈负相关,与TXA2表达水平呈正相关(P<0.05);经单因素分析和多因素Logistic回归分析,高血压史、Hunt-Hess分级、动脉瘤直径、血清Cav-1、TXA2均是aSAH患者预后的独立影响因素(P<0.05);经ROC曲线分析,血清Cav-1联合TXA2预测aSAH患者预后不良的敏感度为89.16%,特异度为48.07%,AUC为0.890。结论:血清Cav-1水平降低和TXA2水平升高与aSAH发病及病情严重程度密切相关,联合应用可提高对预后的评估效能。 |

ABSTRACT Objective: To analyze the expression significance of serum Cav-1 (CAV-1) and thromboxane A2(TXA2) in patients with aneurysmal subarachnoid hemorrhage (aSAH) and their efficacy in evaluating prognosis. Methods: Select our hospital from January 2021 to January 2023, 110 patients with aSAH as observation group, choose 110 cases of healthy physical examination as a control.Detection of all the subjects, serum levels of Cav-1, the expression of TXA2. Using the Spearman correlation analysis aSAH patients serum Cav-1, TXA2 expression level and WFNS grading, Hunt - Hess classification and grade of improved Fisher correlation. The prognosis of aSAH patients was followed up and recorded. Analyze the influencing factors in the prognosis of aSAH patients and evaluate the predictive efficacy of serum Cav-1 combined with TXA 2 in aSAH patients by AUC. Results: Observation group of Cav-1 expression level is lower than the control group, serum TXA2 expression level is higher than the control group(P<0.05). By the Spearman correlation analysis, patients with aSAH WFNS grading, Hunt - Hess classification and grade of improved Fisher and negatively correlated with serum Cav-1 expression level, and TXA2 expression levels were positively correlated(P<0.05). Factors by single factor analysis and multiple Logistic regression analysis, history of hypertension, Hunt - Hess classification, aneurysm diameter, serum Cav-1, TXA2 were independent impact factors for the prognosis of patients with aSAH(P<0.05). ROC curve analysis showed that the sensitivity,specificity and AUC of serum Cav-1 combined with TXA2 to predict the poor prognosis of aSAH patients were 89.16%, 48.07% and 0.890. Conclusion: Serum Cav-1 levels and TXA2 levels are closely associated with the illness and disease severity aSAH, joint application can increase the efficiency of the evaluation of prognosis. |
