摘要 目的:探讨高压氧联合阿替普酶静脉溶栓对急性缺血性脑卒中(AIS)患者脑组织血流灌注状态及血清缺血修饰白蛋白(IMA)、神经丝轻链蛋白(NFL)、闭合蛋白(Occludin)水平的影响。方法:选取我院2022年6月~2023年6月124例AIS患者为研究对象,随机抽签法分为对照组和观察组,各62例,对照组给予阿替普酶静脉溶栓治疗,观察组在对照组基础上给予高压氧治疗。比较两组临床疗效、卒中量表(NHISS)与改良型Rankin(mRS)评分、脑组织血流灌注状态[脑血流量(CBF)、脑血容量(CBV)、平均通过时间(MTT)、达峰反应时间(TMax)]、血清IMA、NFL、Occludin水平及不良反应发生率。结果:相较于对照组,观察组总有效率较高,治疗2周后NHISS、mRS评分降低(P<0.05);相较于对照组,观察组治疗2周后CBF、CBV升高,MTT、TMax缩短(P<0.05);相较于对照组,观察组治疗2周后血清IMA、Occludin、NFL水平降低(P<0.05);两组不良反应发生率无显著差异(P>0.05)。结论:高压氧联合阿替普酶静脉溶栓治疗AIS患者,可改善脑组织血流灌注状态,降低血清IMA、Occludin、NFL水平,减轻神经功能损伤,疗效显著,且安全性高。 |

ABSTRACT Objective: To investigate the effects of hyperbaric oxygen combined with alteplase intravenous thrombolysis on cerebral blood perfusion status and serum ischaemic modified albumin (IMA), neurofilament light chain protein (NFL) and Occludin levels in patients with acute ischemic stroke (AIS). Methods: 124 patients with AIS in our hospital from June 2022 to June 2023 were selected as the study objects, and were randomly divided into control group and observation group, 62 cases in each group. The control group was given alteplase intravenous thrombolysis therapy, and the observation group was given hyperbaric oxygen therapy on the basis of the control group. The clinical efficacy, National institute of Health Stroke Scale (NHISS) and modified Rankin scale (mRS) score, cerebral perfusion status [cerebral blood flow (CBF), cerebral blood volume (CBV), mean transit time (MTT), time to peak(TMax)], serum IMA, NFL, Occludin levels and incidence of adverse reactions were compared between the two groups. Results: Compared with control group, the total effective rate of observation group was higher, NHISS and mRS score were decreased after 2 weeks of treatment (P<0.05). Compared with control group, CBF and CBV were increased and MTT and TMax were shortened in observation group after 2 weeks of treatment(P<0.05). Compared with control group, serum IMA, Occludin and NFL levels in observation group decreased after 2 weeks of treatment (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion: Hyperbaric oxygen combined with alteplase intravenous thrombolytic therapy for AIS patients can improve cerebral blood perfusion, reduce serum IMA, Occludin and NFL levels, and alleviate nerve function injury, with significant efficacy and high safety. |
