| 摘要 目的：基于Toll样受体4（TLR4）/核转录因子-κB（NF-κB）信号通路分析妇科千金胶囊联合头孢地尼治疗慢性盆腔炎的疗效及其机制。方法：根据随机数字表法，将2022年2月~2023年2月期间西北妇女儿童医院收治的慢性盆腔炎患者（n=148）分为对照组（头孢地尼治疗，74例）和研究组（头孢地尼、妇科千金胶囊联合治疗，74例）。对比两组临床症状消失时间、炎症因子水平[肿瘤坏死因子-α（TNF-α）、C反应蛋白（CRP）、白细胞介素-8（IL-8）]、血液流变学指标和TLR4/NF-κB信号通路相关指标。结果：研究组的腹部坠胀、疼痛缓解时间和腰骶部酸痛缓解时间均短于对照组（P<0.05）。治疗后两组CRP、TNF-α、IL-8下降，且研究组较对照组低（P<0.05）。两组治疗后红细胞聚集指数、全血高切/低切黏度、纤维蛋白原下降，且研究组低于对照组（P<0.05）。两组治疗后TLR4、髓样分化因子88（MyD88）、NF-κB下降，且研究组低于对照组（P<0.05）。结论：慢性盆腔炎患者采用妇科千金胶囊联合头孢地尼治疗，可改善临床症状，调节炎症因子水平和血液流变学水平，可能与调节TLR4NF-κB信号通路有关。 |

| ABSTRACT Objective: To analyze the efficacy and mechanism of fuke qianjin capsules combine with cefdinir in treatment of chronic pelvic inflammatory disease based on Toll-like receptor 4 (TLR4)/nuclear transcription factor-κB (NF-κB) signaling pathway. Methods: According to the random number table method, patients with chronic pelvic inflammatory disease (n=148) who were admitted to Northwest Women and Children's Hospital from February 2022 to February 2023 were divided into control group (cefdinir treatment, 74 cases) and study group (cefdinir combine with fuke qianjin capsules, 74 cases). The disappearance time of clinical symptoms, levels of inflammatory factors [tumor necrosis factor-α (TNF-α), C-reactive protein (CRP), interleukin-8 (IL-8)], hemorheological indexes and TLR4/NF-κB signaling pathway related indexes were compared between two groups. Results: The abdominal bulge, pain relief time and lumbosacral pain relief time in study group were shorter than those in control group (P<0.05). CRP, TNF-α and IL-8 in two groups decreased after treatment, and study group was lower than that of control group (P<0.05). Erythrocyte aggregation index, whole blood high shear/low shear viscosity and fibrinogen decreased in two groups after treatment, and study group was lower than that of control group (P<0.05). TLR4, myeloid differentiation factor 88 (MyD88) and NF-κB decreased in two groups after treatment, and study group wase lower than that of control group (P<0.05). Conclusion: The patients with chronic pelvic inflammatory disease treated with Fuke qianjin capsules combined with cefdinir, which can can improve clinical symptoms, regulate the levels of inflammatory factors and hemorheology, and may be relate to the regulation of TLR4 NF-κB signaling pathway. |
