| 摘要 目的：探讨益气化瘀汤对药物流产后恶露不绝气虚血瘀证患者子宫机能、凝血功能和血液流变学的影响。方法：按照随机数字表法将2020年6月-2021年6月期间河北省中医院收治的80例药物流产后恶露不绝气虚血瘀证患者分为联合组（n=40，对照组基础上接受益气化瘀汤治疗）和对照组（n=40，常规西医治疗）。比较两组中医证候积分、止血率、止血时间、月经复潮时间、子宫机能、凝血功能和血液流变学变化情况。结果：与对照组相比，联合组治疗7 d后恶露量、恶露质、恶露色、小腹疼痛、少气懒言、四肢乏力评分更低，子宫动脉收缩期峰值流速（PSV）、止血率更高，月经复潮时间、止血时间更短，搏动指数（PI）、阻力指数（RI）、子宫三径之和、活化部分凝血活酶时间（APTT）、全血高切黏度（HS）、全血低切黏度（LS）、凝血酶原时间（PT）、血沉（ESR）、纤维蛋白原（FIB）、血细胞比容（Hct）、凝血酶时间（TT）更低（P<0.05）。结论：益气化瘀汤用于药物流产后恶露不绝气虚血瘀证患者，具有较好的疗效，可有效改善子宫机能、血液流变学、凝血功能。 |

| ABSTRACT Objective: To explore the effect of yiqihuayu decoction on uterine function, coagulation function and hemorheology in patients with lochiorrhea after drug abortion with qi deficiency and blood stasis syndrome. Methods: 80 patients with lochiorrhea after drug abortion with qi deficiency and blood stasis syndrome who were admitted to Hebei Provincial Hospital of Traditional Chinese Medicine from June 2020 to June 2021 were divided into combination group (n=40, treated with yiqihuayu decoction on the basis of control group) and control group (n=40, treated with conventional Western medicine) according to the random number table method. The traditional chinese medicine syndrome scores, hemostasis rate, hemostasis time, menstrual recovery time, uterine function, coagulation function and hemorheology changes were compared between two groups. Results: Compared with control group, the scores of lochia volume, lochia substance, lochia color, lower abdominal pain, lack of qi and lazy speech and limb fatigue were lower in combination group 7d after treatment, the peak systolic velocity (PSV) and hemostasis rate of the uterine artery were higher, the menstrual recovery time and hemostasis time were shorter, the pulsatility index (PI), resistance index (RI), sum of three uterine diameters, activated partial thromboplastin time (APTT), whole blood high shear viscosity (HS), whole blood low shear viscosity (LS), prothrombin time (PT), erythrocyte sedimentation rate (ESR), fibrinogen (FIB), hematocrit (Hct), thrombin time (TT) were lower (P<0.05). Conclusion: Yiqihuayu decoction is used for patients with lochiorrhea after drug abortion with qi deficiency and blood stasis syndrome, which has good therapeutic effects, and can effectively improve uterine function, hemorheology and coagulation function. |
