摘要 目的:探讨不稳定型心绞痛患者血清氧化修饰低密度脂蛋白(ox-LDL)及低氧诱导因子-1α(HIF-1α)的表达意义及其与冠状动脉病变严重程度的关系。方法:选取我院2020年3月到2023年1月收治的100例不稳定型心绞痛患者作为研究对象,分析其冠状动脉造影诊断结果,应用Gensini评分法对患者的冠状动脉病变严重程度进行评价,将≤20分的30例患者分为轻度病变组,20~50分的45例患者分为中度病变组,≥50分的25例患者分为重度病变组。对比三组患者血清ox-LDL及HIF-1α表达水平,并分析血清ox-LDL及HIF-1α与冠状动脉病变严重程度的相关性;依照PCI术后是否发生不良心血管事件(MACE)分为非MACE组(n=79)和MACE组(n=21),对比两组患者临床特征及血清ox-LDL及HIF-1α的表达水平,并应用logistic回归分析分析血清ox-LDL及HIF-1α对不稳定型心绞痛患者不良心血管事件的预测价值。结果:不同严重程度的三组患者血清ox-LDL及HIF-1α表达水平对比差异显著,重度病变组ox-LDL、HIF-1α水平明显高于中度病变组和轻度病变组(P<0.05),而中度病变组与轻度病变组对比无统计学意义(P>0.05);Spearman相关分析结果显示:HIF-1α、ox-LDL与不稳定型心绞痛冠状动脉病变严重程度呈正相关(P<0.05);两组患者心功能分级、左室射血分数、TIMI评分、血清ox-LDL及HIF-1α水平对比差异显著(P<0.05);logistic回归分析结果表明:ox-LDL及HIF-1α为不稳定型心绞痛PCI介入术后不良心血管事件的独立危险因素(P<0.05)。结论:血清ox-LDL及HIF-1α水平与不稳定型心绞痛冠状动脉病变严重程度具有明显相关性,且ox-LDL及HIF-1α为不稳定型心绞痛患者介入手术后近期主要心血管事件的独立危险因素。临床上对于HIF-1α、ox-LDL升高的不稳定型心绞痛患者应注意及时采取相关措施预防近期心血管不良事件的发生。 |

ABSTRACT Objective: To explore the serum oxidized low density lipoprotein (ox LDL) and hypoxia inducible factor-1 in patients with unstable angina pectoris α (HIF-1α). The expression significance and its relationship with the severity of coronary artery disease. Methods: 100 patients with unstable angina pectoris admitted to our hospital from March 2020 to March 2023 were selected as the research subjects. The results of coronary angiography diagnosis were retrospectively analyzed, and the severity of coronary artery disease was evaluated using the Gensini scoring method. 30 patients with a score of less than 20 were divided into a mild disease group, 45 patients with a score of 20-50 were divided into a moderate disease group, and 25 patients with a score of more than 50 were divided into a severe disease group. Results: The serum expression levels of ox-LDL and HIF-1 α were significantly different between the three groups of patients with different degrees of severity, and the levels of ox-LDL and HIF-1 α were significantly higher than the moderate lesion group and mild lesion group(P<0.05), while the moderate lesion group and mild lesion group were not significant(P>0.05); Spearman's correlation analysis showed that HIF-1α and ox-LDL were positively correlated with the severity of coronary artery lesions in unstable angina(P<0.05). The significant difference between the two groups in terms of cardiac function class, left ventricular ejection fraction, TIMI score, serum ox-LDL and HIF-1α levels (P<0.05); Logistic regression analysis The results showed that ox-LDL and HIF-1α were independent risk factors for adverse cardiovascular events after PCI intervention for unstable angina(P<0.05). Conclusion: Serum ox-LDL and HIF-1 α There is a significant correlation between levels and the severity of coronary artery disease in unstable angina pectoris, and ox-LDL and HIF-1α An independent risk factor for major cardiovascular events in patients with unstable angina after interventional surgery.
