| 摘要 目的：探讨丹参川芎嗪注射液联合依达拉奉治疗急性缺血性脑卒中合并高血压的疗效及对神经功能、血脂和脑源性神经营养因子（BDNF）、半乳糖凝集素3（GAL3）的影响。方法：选择我院自2020年1月至2022年1月接诊的136例急性缺血性脑卒中合并高血压患者作为研究对象，分为对照组和观察组，各68例。两组均在常规治疗的基础上，对照组加用依达拉奉治疗，观察组加用丹参川芎嗪注射液联合依达拉奉治疗。比较两组治疗前后的脑血流动力学指标、美国国立卫生研究院卒中量表（NIHSS）评分、改良Barthel（BMI）评分、血脂、血清BDNF、GAL3表达水平，综合评价临床疗效，记录不良反应发生情况。结果：观察组治疗后峰流速（Vp）、平均流速（Vm）均大于对照组，峰流速对称性差值（DVp）、平均流速对称性差值（DVm）均小于对照组（P<0.05）；观察组治疗后NIHSS评分较对照组低，BMI评分较对照组高（P<0.05）；观察组治疗后血脂水平较对照组低（P<0.05）；观察组治疗后血清BDNF表达水平高于对照组，GAL3表达水平低于对照组（P<0.05）；观察组临床总显效率较对照组高（P<0.05）；两组不良反应发生率比较，差异不显著（P>0.05）。结论：丹参川芎嗪注射液联合依达拉奉治疗急性缺血性脑卒中合并高血压的疗效确切，能够有效改善神经功能，调节血脂代谢和升高BDNF水平、降低GAL3水平，且安全性较好，值得临床予以重视。 |

| ABSTRACT Objective: To investigate the efficacy of Salvia ligustrazine injection combined with Edaravone in the treatment of acute ischemic stroke complicated with hypertension and the effects on nerve function, blood lipid, brain-derived neurotrophic factor(BDNF) and galactose lectin 3(GAL3). Methods: A total of 136 patients with acute ischemic stroke complicated with hypertension admitted to our hospital from January 2020 to January 2022 were selected as research objects and randomly divided into matched group and observation group, with 68 cases in each group. On the basis of conventional treatment, the matched group was treated with edaravone, and the observation group was treated with Danshen Chuangxiong Azine injection combined with Edaravone. The cerebral hemodynamic indexes, National Institutes of Health Stroke Scale(NIHSS) score, modified Barthel(BMI) score, blood lipid, serum BDNF, GAL3 expression levels of the two groups before and after treatment were compared, the clinical efficacy was comprehensively evaluated, and the occurrence of adverse reactions was recorded. Results: After treatment, the peak flow velocity (Vp) and average flow velocity (Vm) of the observation group were higher than those of the matched group, and the symmetry difference of peak flow velocity (DVp) and average flow velocity symmetry difference (DVm) of the observation group were lower than those of the matched group(P<0.05). After treatment, NIHSS score of observation group was lower than that of matched group,and BMI score was higher than that of matched group(P<0.05); After treatment, the levels of total cholesterol (TC), triglyceride (TG) and low-density lipoprotein cholesterol(LDL-C) in observation group were lower than those in matched group (P<0.05); After treatment, the serum BDNF expression level of the observation group was higher than that of the matched group, and the GAL3 expression level was lower than that of the matched group(P<0.05). The total clinical effective rate of observation group was higher than that of matched group (P<0.05). There was no difference in the incidence of adverse reactions between the two groups(P>0.05). Conclusion: Salviorrhiza Chuanxiustrazine injection combined with Edaravone has definite efficacy in the treatment of acute ischemic stroke complicated with hypertension, which can effectively improve nerve function, regulate lipid metabolism, increase BDNF level and reduce GAL3 level, and is safe and reliable, which is worthy of attention. |
