| 摘要 目的：探讨祛瘀消肿汤联合骨康胶囊在老年桡骨远端骨折（DFA）术后患者中的应用效果。方法：选择2016年1月-2020年12月在我院接受治疗的老年DFA患者，共为80例。采用随机数字表法分为对照组40例（接受骨康胶囊治疗），研究组40例（在对照组的基础上接受祛瘀消肿汤治疗）。对比两组疗效、关节活动度、骨代谢标志物、临床症状、不良反应情况。结果：研究组的优良率高于对照组（P<0.05）。两组治疗1个月后外展、背伸、掌屈、桡偏角度活动范围扩大，且研究组大于对照组（P<0.05）。两组不良反应发生率组间对比无差异（P>0.05）。两组治疗1个月骨钙素（BGP）、骨保护素（OPG）、碱性磷酸酶（ALP）升高，抗酒石酸酸性磷酸酶5b（TRACP5b）、Ⅰ型胶原交联羧基末端肽（CTX-Ⅰ）、核转录因子-κＢ受体活化因子（RANK）下降，且研究组的变化幅度大于对照组（P<0.05）。研究组的自觉疼痛完全缓解时间、骨折愈合时间、腕关节肿胀消退时间短于对照组（P<0.05）。结论：祛瘀消肿汤联合骨康胶囊治疗老年DFA术后患者，可促进骨折愈合，改善关节活动度，促进患者术后恢复，效果明显。 |

| ABSTRACT Objective: To investigate the application effect of Quyu Xiaozhong decoction combined with Gukang capsule in elderly patients with distal radius fracture (DFA) after surgery. Methods: 80 elderly patients with DFA who were treated in our hospital from January 2016 to December 2020 were selected. The patients were randomly divided into control group with 40 cases (received Gukang capsule treatment) and study group with 40 cases (received Quyu Xiaozhong decoction treatment on the basis of control group). The efficacy, range of motion, bone metabolism markers, clinical symptoms and adverse reactions were compared between the two groups. Results: The excellent and good rate of the study group was higher than that of the control group(P<0.05). 1 month after treatment, the range of motion of abduction, dorsiflexion, palm flexion and radial deviation angle of the two groups were expanded, and the study group was greater than the control group (P<0.05). There was no difference in the incidence of adverse reactions between the two groups (P>0.05). 1 month after treatment, osteocalcin (BGP), osteoprotegerin (OPG) and alkaline phosphatase (ALP) of the two groups were increased, tartrate-resistant acid phosphatase 5b (TRACP5b), cross-linked carboxy-terminal peptide of type Ⅰ collagen (CTX-Ⅰ) and receptor activator of nuclear factor-κB (RANK) in the two groups were decreased, the amplitude of change in the study group was greater than that in the control group(P<0.05). The complete pain relief time, fracture healing time and wrist swelling resolution time in the study group were shorter than those in the control group(P<0.05). Conclusion: Quyu Xiaozhong decoction combined with Gukang capsule in the treatment of elderly patients with DFA after surgery can promote fracture healing, improve joint range of motion, and promote postoperative recovery of patients, with obvious effect. |
