| 摘要 目的：探讨外周血中性粒细胞/淋巴细胞比值（NLR）、血清白细胞介素-6（IL-6）及鼻窦CT评分诊断慢性鼻窦炎伴鼻息肉（CRSwNP）的价值。方法：选择2019年2月至2021年12月济宁医学院附属医院收治的91例CRSwNP患者纳入观察组，87例鼻中隔偏曲患者纳入对照组，根据《中国慢性鼻窦炎诊断与治疗指南（2018）》将CRSwNP患者分为轻度组（29例）、中度组（40例）和重度组（22例）。所有受试者均检测外周血中性粒细胞、淋巴细胞计数以及血清IL-6水平，计算外周血NLR，行鼻窦CT检查，根据CT检测结果，采用Lund-Mackay CT评分标准对受试者进行鼻窦CT评分。对比各组外周血NLR、血清IL-6水平及鼻窦CT评分差异，分析CRSwNP 患者外周血NLR、血清IL-6与鼻窦CT评分的相关性以及外周血NLR、血清IL-6、鼻窦CT评分诊断CRSwNP的价值。结果：观察组外周血NLR、血清IL-6水平及鼻窦CT评分均高于对照组（P＜0.05）。重度组外周血NLR、血清IL-6水平及鼻窦CT评分均高于中度组和轻度组（P＜0.05），且中度组高于轻度组（P＜0.05）。CRSwNP患者外周血NLR、血清IL-6水平与鼻窦CT评分均呈正相关（r=0.502、0.539，P＜0.05）。外周血NLR、血清IL-6及鼻窦CT评分诊断CRSwNP的曲线下面积（AUC）分别为0.683、0.659、0.697，联合三项指标诊断CRSwNP的AUC为0.882，高于各指标单独诊断。结论：外周血NLR、血清IL-6水平及鼻窦CT评分上升可反映CRSwNP患者病情加重，外周血NLR、血清IL-6水平与鼻窦CT评分呈正相关，外周血NLR、血清IL-6及鼻窦CT评分联合诊断CRSwNP具有一定的临床价值。 |

| ABSTRACT Objective: To investigate the value of peripheral blood neutrophil/lymphocyte ratio (NLR), serum interleukin-6 (IL-6) and paranasal sinus CT score in the diagnosis of chronic sinusitis with nasal polyps (CRSwNP). Methods: From February 2019 to December 2021, 91 patients with CRSwNP who were treated in the Affiliated Hospital of Jining Medical College were included in the observation group, and 87 patients with nasal septum deviation were included in the control group. According to the Chinese guidelines for the diagnosis and treatment of chronic sinusitis (2018), patients with CRSwNP were divided into mild group (29 cases), moderate group (40 cases) and severe group (22 cases). All subjects were tested for peripheral blood neutrophils, lymphocyte counts and serum IL-6 levels, calculated peripheral blood NLR, and they were underwent paranasal sinus CT examination, according to the CT results, the paranasal sinus CT scores were performed using Lund-Mackay CT scoring criteria. The levels of peripheral blood NLR, serum IL-6 and paranasal sinus CT scores differences in each group were compared. The correlation between peripheral blood NLR, serum IL-6 and paranasal sinus CT scores in patients with CRSwNP and the value of peripheral blood NLR, serum IL-6 and paranasal sinus CT scores in the diagnosis of CRSwNP were analyzed. Results: The peripheral blood NLR, serum IL-6 levels and paranasal sinus CT score in the observation group were higher than those in the control group(P<0.05). The peripheral blood NLR, serum IL-6 levels and paranasal sinus CT score in severe group were higher than those in moderate group and mild group(P<0.05), and the moderate group was higher than the mild group(P<0.05). The peripheral blood NLR, serum IL-6 levels of patients with CRSwNP were positively correlated with the paranasal sinus CT score (r=0.502, 0.539, P<0.05). The area under the curve (AUC) of peripheral blood NLR, serum IL-6 and paranasal sinus CT score in the diagnosis of CRSwNP were 0.683, 0.659 and 0.697 respectively. The AUC of combined three indexes in the diagnosis of CRSwNP was 0.882, which was higher than that of each index alone. Conclusion: The increase of peripheral blood NLR, serum IL-6 and paranasal sinus CT score can reflect the aggravation of patients with CRSwNP. The peripheral blood NLR and serum IL-6 levels are positively correlated with paranasal sinus CT score. The combination of peripheral blood NLR, serum IL-6 and paranasal sinus CT score has certain clinical value in the diagnosis of CRSwNP. |
