      摘要 目的:探讨六味地黄丸组方对前交叉韧带损伤(ACL)损伤患者 ACL组织转化生长因子 -β1(TGF-β1)、骨形态发生蛋白 -2(BMP-2)基因表达及 ACL重建术后功能康复的影响。方法:选取 2021年 1月 -2021年 9月期间在广东省中医院珠海医院骨一科住院的拟行膝关节镜下 ACL重建术治疗的 ACL损伤患者 60例,按随机数字表法分为观察组和对照组,每组 30例。观察组术前2周开始口服六味地黄丸组方,且术后继续服用六味地黄丸组方至少 1月,对照组不予服用药物。术中采集患者损伤之前交叉韧带组织,实时荧光定量 PCR(RT-qPCR)检测韧带组织的 TGF-β1和 BMP-2基因表达,并对患者进行 IKDC评分、Lysholm评分以及安全性评价。结果:观察组 TGF-β1基因表达高于对照组,差异有统计学意义(P<0.05)。两组术前 IKDC评分和 Lysholm评分比较差异均无统计学意义(均P>0.05)。两组 IKDC评分和 Lysholm评分与治疗时间之间有显著性交互效应(均P<0.01),两组IKDC评分和 Lysholm评分总体均值有显著性差异(均P<0.01)。两组术后 1周 -术后 1月,术后 1月 -术后 3月 IKDC评分和Lysholm评分有显著性差异(均P=0.000);与对照组比较,治疗组术后 1月、术后 3月 IKDC评分和 Lysholm评分有显著性差异(均P=0.000)。两组患者在治疗期间均未发生不良事件。结论:六味地黄丸组方可提高 ACL损伤患者腱骨界面处 ACL组织 TGF-β1基因表达,促进 ACL重建术后关节功能康复。

      ABSTRACT Objective: To investigate the effect of Liuwei Dihuang Pill Prescription on transforming growth factor-β1(TGF-β1) and bone morphogenetic protein-2 (BMP-2) gene expression in ACL tissue and functional recovery rehabilitation after ACL reconstruction in patients with anterior cruciate ligament(ACL)injury. Methods: A total of 60 patients with ACL injury who were hospitalized in the First Department of Orthopedics, Zhuhai Hospital of Guangdong Province Traditional Chinese Medical Hospital from January 2021 to September 2021 and were to undergo knee arthroscopy with ACL reconstruction were selected and divided into observation group and control group by random number table method, 30 cases in each group. The observation group started to take Liuwei Dihuang Pill Prescription 2 weeks before operation, and continued to take Liuwei Dihuang Pill Prescription for at least 1 month after the operation, while the control group did not take drugs. The cruciate ligament tissue before the injury was collected during the operation, and the gene expression of TGF-β1 and BMP-2 in the ligament tissue was detected by real-time fluorescence quantitative PCR (RT-qPCR). Results: The expression of TGF-β1 gene in the observation group was higher than that in the control group, and the difference was statistically significant (P<0.05). There was no significant difference in preoperative IKDC score and Lysholm score between the two groups (allP>0.05). There was a significant interaction effect between the two groups of IKDC score and Lysholm score and treatment time (allP<0.01), and there was a significant difference in the overall mean of IKDC score and Lysholm score between the two groups (allP<0.01). There were significant differences in IKDC score and Lysholm score between the two groups from 1 week to 1 month after operation, and from 1 month to 3 months after operation (allP=0.000). Compared with the control group, there were significant differences in the IKDC score and Lysholm score of the treatment group at 1 month and 3 months after the operation (allP=0.000). No adverse events occurred in the two groups of patients during the treatment period. Conclusion: Liuwei Dihuang Pills Prescription can increase the expression of TGF-β1 gene in ACL tissue at the tendon-bone interface of patients with ACL injury,and promote joint functional recovery after ACL reconstruction.
