      摘要 目的:探索外周血嗜酸性粒细胞比率、血小板平均体积(MPV)、血小板平均宽度(PDW)、大型血小板比率(P-LCT)与重度哮喘患者肺功能及哮喘发作的关系。方法:回顾2017年1月至2018年12月于我院就诊,首次经肺功能检查,诊断为支气管哮喘患者 122例,其中重度患者(SAG) 56例;轻中度患者(n-SAG) 66例。选择同期在我院行健康体检志愿者(CP)的 37名,作为对照组。通过医院HIS系统调取研究对象的年龄、性别,BMI指数等一般资料,调取WBC、EOS%、PLT、PDW、MPV、P-LCT等血液结果,调取FVC pre 、FEV1 pre 、FVC%、FEV1%、FVC post 、FEV1 post 等肺功能检查结果。采用单因素方差分析、Pearson相关分析、Logistic 回归分析等方法对相关指标进行统计分析。结果:正常组的PDW、MPV、P-LCT水平均低于重症哮喘组,具有统计学意义(均P <0.05), 正常组的EOS%水平明显低于重症哮喘组,具有显著统计学意义(P <0.01),重症哮喘患者FVC pre 、FEV1 pre 、FVC%、FEV1%、FVC post 、FEV1 post 均显著低于非重症患者及观察组,具有显著统计学意义(均P <0.01)。Pearson相关性分析显示,EOS%与FEV1改善量具有相关性(r=0.233,P=0.003)。PDW、MPV、P-LCT与FVC改善量均具有显著相关性(均P<0.01), PDW、MPV、P-LCT与FVC变异率、FEV1变异率具有相关性(均P<0.05)。嗜酸性粒细胞与血小板比率(EPR)与FEV1改善量具有相关性(r=0.159,P=0.048)logistic回归分析显示:EOS(Exp(B)=1.203)、P-LCT(Exp(B)=1.086)与哮喘发作密切相关。结论:外周血EOS%、PDW、MPV和P-LCT水平与哮喘肺功能具有密切相关性,在判断哮喘发作风险及哮喘发作严重程度反面具有一定参考作用。

      ABSTRACT Objective: To investigate the relationship between Eosinophil ratio(EOS%), platelet distribution width(PDW), mean platelet volume(MPV), platelet-larger cell ratio(P-LCT)and lung function in patients with asthma. Methods: A total of 122 patients with asthma who were treated in our hospital from January 2017 to December 2018, were recruited as study objects, according to the status of lung function, 56 patients were assigned as severe asthma group(SAG), and 66 patients were assigned as non-severe asthma group(non-SAG). Meanwhile, 37 healthy subjects who underwent physical examination in our hospital during the same period were selected as the control group(CG). Age, gender, BMI, blood routine examination, lung function results of the research subjects were collected in the HIS system. The level of WBC, EOS%, PDW, MPV, P-LCT and lung function were compared among the three groups. Pearson correlation analysis and regression analysis were used for correlation analysis of all indicators. Logistic regression analysis was used to screen the relevant factors of asthma attacks. Results: Compared with SAG, the level of PDW, MPV, P-LCT in CG, were different( P <0.05). Compared with SAG, the level of EOS% of CG was significantly different(P <0.01). The results of FVC pre , FEV1 pre , FVC%, FEV1%, FVC post , FEV1 post were significant different among the three groups (P <0.01). Pearson correlation analysis showed that EOS% was correlated with change in FEV1. PDW, MPV, P-LCT were correlated with change in FVC, change in FVC ratio, change in FEV1 ratio. Logistic regression analysis showed that EOS%(Exp(B)=1.203), P-LCT(Exp(B)=1.086) were risk factors of asthma attack. Conclusion: The levels of EOS%, PDW, MPV and P-LCT are closely related to pulmonary function in patient with asthma, and have a certain clinical value in diagnosis of severe asthma.
