摘要 目的:探讨新诊断2型糖尿病患者血清缝隙连接蛋白-1(Pannexin-1)、爱帕琳肽(Apelin)、视黄醇结合蛋白4(RBP4)、血管内皮生长因子(VEGF)水平与糖脂代谢和胰岛素抵抗的关系。方法:选择2018年2月至2020年2月我院诊治的120例新诊断2型糖尿病患者作为糖尿病组,选择同期在我院进行体检的120例健康者作为对照组。检测所有受试者甘油三酯(TG)、总胆固醇(TC)、谷草转氨酶(AST)、谷丙转氨酶(ALT)、高密度脂蛋白(HDL)和低密度脂蛋白(LDL)水平、空腹血糖(FPG)、空腹胰岛素(FINS)、Pannexin-1、Apelin、RBP4、VEGF水平,计算胰岛素抵抗指数(HOMA-IR)、胰岛素敏感性指数(ISI)和胰岛?茁细胞功能指数(HOMA-β)。分析Pannexin-1、Apelin、RBP4、VEGF水平与糖脂代谢和胰岛素抵抗相关指标的关系,分析影响上述指标表达的相关因素。结果:与对照组相比,糖尿病组体质量指数(BMI)、TG、TC、FPG、FINS、Pannexin-1、Apelin、RBP4、VEGF水平,HOMA-IR明显升高(P<0.05),ISI和HOMA-β明显下降(P<0.05)。新诊断2型糖尿病患者血清Pannexin-1、Apelin、RBP4、VEGF水平与ISI和HOMA-β均呈负相关(P<0.05),与BMI、TG、TC、FPG、FINS、HOMA-IR均呈正相关(P<0.05)。TG与Pannexin-1表达联系密切(β=0.006,P<0.05),ISI和HOMA-IR与Apelin表达联系密切(β=-6.673、0.049,P<0.05),FPG和TC与RBP4表达联系密切(β=22.309、0.506,P<0.05),HOMA-IR与VEGF表达联系密切(β=0.574,P<0.05)。结论:新诊断2型糖尿病患者血清Pannexin-1、Apelin、RBP4、VEGF水平异常升高,并参与新诊断2型糖尿病患者的糖脂代谢和胰岛素抵抗,在2型糖尿病的诊断和预防中有一定临床价值。 |

ABSTRACT Objective: To investigate the relationship between serum levels of Pannexin-1 (Pannexin-1), Apelin (Apelin), retinol binding protein (RBP4) and vascular endothelial growth factor (VEGF), glucose and lipid metabolism and insulin resistance in newly diagnosed type 2 diabetes patients. Methods: 120 newly diagnosed patients with type 2 diabetes diagnosed and treated in our hospital from February 2018 to February 2020 were selected as the diabetes group, and 120 healthy patients who underwent physical examination in our hospital during the same period were selected as the control group. The levels of triglyceride (TG), total cholesterol (TC), aspartame aminotransferase (AST), aspartame aminotransferase (ALT), high-density lipoprotein (HDL) and low-density lipoprotein (LDL), fasting blood glucose (FPG), fasting insulin (FINS), pannexin-1, Apelin, RBP4 and VEGF in all subjects were measured. Insulin resistance index (HOMA-IR), insulin sensitivity index (ISI), and islet insulin function index (HOMA-β) were calculated. The relationship between the levels of pannexin-1, Apelin, RBP4 and VEGF and the indicators related to glucose and lipid metabolism and insulin resistance were analyzed, and the significant factors affecting the expression of these indicators were analyzed. Results: Compared with the control group, the body mass index (BMI), TG, TC, FPG, FINS, Pannexin-1, Apelin, RBP4 and VEGF levels, HOMA-IR in the diabetes group were significantly increased (P<0.05), and the ISI and HOMA-β were significantly decreased (P<0.05). Serum Pannexin-1, Apelin, RBP4 and VEGF were negatively correlated with ISI and HOMA-β in newly diagnosed type 2 diabetes patients (P<0.05), and positively correlated with BMI, TG, TC, FPG, FINS and HOMA-IR (P<0.05). TG was closely correlated with the expression of Pannexin-1 (β=0.006, P<0.05), ISI and HOMA-IR were closely correlated with the expression of Apelin (β=-6.673, 0.049; P<0.05), FPG and TC were closely correlated with the expression of RBP4 (β=22.309, 0.506; P<0.05), and HOMA-IR was closely correlated with the expression of VEGF (β=0.574, P<0.05). Conclusion: Serum Pannexin-1, Apelin, RBP4 and VEGF levels are abnormally elevated in patients with newly diagnosed type 2 diabetes, and they are involved in glucose and lipid metabolism and insulin resistance in patients with newly diagnosed type 2 diabetes, which have certain clinical value in the diagnosis and treatment of type 2 diabetes. |
