摘要 目的:探讨基质金属蛋白酶、血清白介素及多肽生长因子与急性脑梗死患者颈动脉斑块的关系。方法:选取于2017年1月至2017年12月期间北京中医药大学第三附属医院老年病科收治的60 例急性脑梗死患者作为观察组,根据彩色多普勒超声检测仪进行颈动脉超声检查,将结果分为无斑块组(20 例),稳定斑块组(20 例)和不稳定斑块组(20 例),选取同期的60 例门诊体检健康者作为对照组。采用双抗体夹心酶标免疫吸附法(ELISA)测定血清基质金属蛋白酶MMP-7、MMP-9、MMP-12水平,血清白介素IL-6、IL-8、IL-18以及血清多肽生长因子VEGF和TGF-β1水平,对观察组及对照组的检测结果以及无斑块组、稳定斑块组、不稳定斑块组及对照组的检测结果进行分析研究。结果:观察组的血清基质金属蛋白酶MMP-7、MMP-9、MMP-12水平,血清白介素IL-6、IL-8、IL-18水平以及血清多肽生长因子VEGF水平显著高于对照组,差异有统计学意义(P<0.05),观察组的血清多肽生长因子TGF-β1水平则显著低于对照组,差异有统计学意义(P<0.05)。稳定性斑块组体质指数显著高于对照组(P<0.05);不稳定性斑块组与对照组相比,BMI、TC及TG水平均显著升高(P<0.05),且与无斑块组相比,甘油三酯水平也偏高(P<0.05)。无斑块组,稳定斑块组及不稳定斑块组的血清基质金属蛋白酶MMP-7、MMP-9、MMP-12水平,血清白介素IL-6、IL-8、IL-18水平以及血清多肽生长因子VEGF和TGF-β1水平比较,差异有统计学意义(P<0.05),其中除血清多肽生长因子TGF-β1水平显著低于其它2组外,不稳定斑块组的血清基质金属蛋白酶MMP-7、MMP-9、MMP-12水平,血清白介素IL-6、IL-8、IL-18水平以及血清VEGF水平显著高于其它2组,差异有统计学意义(均P<0.05)。结论:急性脑梗死患者的血清基质金属蛋白酶、血清白介素以及血清多肽生长因水平与颈动脉斑块密切相关。 |

ABSTRACT Objective: To explore the relationship of carotid artery plagues and matrix metalloproteinase, interleukin, polypeptide growth factor levels in patients with acute cerebral infraction. Methods: 60 patients with acute cerebral infraction in the department of geriatrics of the Third Affiliated Hospital of Beijing University of Chinese Medicine from January 2017 to December 2017 were selected as the observation group and divided into three subgroups, the no carotid plaques (n=20), the stable plaque (n=20), and the vulnerable plaques (n=20) with carotid ultra-sonographic methods. 60 healthy cases from outpatient were selected as the control group. The matrix metalloproteinase of MMP-7, MMP-9, MMP-12, interleukin of IL-6, IL-8, IL-18, and polypeptide growth factor of VEGF, TGF-β1 levels of all patients with acute cerebral infraction and healthy controls were measured by enzyme linked immunosorbent assay (ELISA). Then the results of all groups were compared. Results: The level of matrix metalloproteinase of MMP-7, MMP-9, MMP-12, interleukin of IL-6, IL-8, IL-18, and polypeptide growth factor of VEGF was higher than that in control group, with significant difference (P<0.05). While the content of polypeptide growth factor of TGF-β1 was lower than the control group, with significant difference (P<0.05). The body mass index of the stable plaque group was significantly higher than that of the control group (P<0.05). The BMI, TC and TG levels were significantly higher in the unstable plaque group than in the control group (P<0.05), and with no plaque. Compared with the block group, the triglyceride level was also high (P<0.05). In the no carotid plaque groups, the stable plaque groups, and the vulnerable plaque groups, the level of matrix metalloproteinase, interleukin and polypeptide growth factor was statistically significant (P<0.05). Except for serum polypeptide growth factor TGF-β1, it was significantly lower than that of the other 2 groups. The levels of serum matrix metalloproteinase of MMP-7, MMP-9, MMP-12, serum interleukin of IL-6, IL-8, IL-18 and serum VEGF were significantly higher in the unstable plaque group than in the other 2 groups.The difference was statistically significant (P>0.05). Conclusion: Serum matrix metalloproteinases, interleukin and polypeptide growth factors are closely related to carotid plaques in patients with acute cerebral infarction. |
