摘要 目的:探讨高原地区高血压患者红细胞指标和血清高迁移率族蛋白2(HMGB2)与颈动脉粥样硬化的相关性。方法:本研究共纳入244例高原地区高血压患者,通过检测颈动脉内膜中层厚度(IMT)分三组:内膜正常组(0.8 mm≤IMT<1.0 mm);内膜增厚组(1.0 mm ≤IMT< 1.2 mm);斑块形成组(IMT≥1.2 mm)。收集患者的临床资料,检测患者红细胞指标和血清HMGB2水平及其他生化指标。通过分析IMT与各项指标之间的相关性,评价颈动脉硬化的相关因素。结果:三组间红细胞压积(HCT)、红细胞计数(RBC)、红细胞分布宽度(RDW)、血清HMGB2水平差异均具有统计学意义(均P<0.05)。相关分析结果显示,RBC(r=0.368)、HCT(r=0.409)、RDW(r=0.596)、HMGB2(r=0.291)与IMT呈正相关(均P<0.05)。多因素Logistic回归分析显示RBC、HCT、RDW、HMGB2是颈动脉硬化的独立危险因素。结论:高原地区高血压患者RBC、HCT、RDW、HMGB2水平与IMT密切相关,且RBC、HCT、RDW、HMGB2水平升高是颈动脉硬化发生的危险因素,可作为动脉硬化早期预测因子。 |

ABSTRACT Objective: To investigate the relationship between indexes of red blood cells and serum high mobility group box-2 (HMGB2) and carotid atherosclerosis in hypertension patients from plateau area. Methods: 224 hypertensive patients from plateau area underwent carotid ultrasonography, and were divided into three groups according to the carotid intima-media thickness (IMT), normal intima group(0.8 mm ≤ IMT< 1.0 mm), intimal thickening group(1.0 mm ≤IMT<1.2 mm), plaque formation group (IMT≥1.2 mm). Clinical data, indexes of erythrocyte, serum HMGB2 levels, and biochemical indexes were examined.Correlation between each index and IMT were analyzed, and the risk factors influencing the formation of carotid atherosclerosis plaque were analyzed by logistic regression. Results: The levels of red blood cell (RBC) count, hematocrit (HCT), red blood cell distribution width (RDW), and HMGB2 in three groups were significantly different from each other(P<0.05, respectively). Results of correlation analysis showed the correlation of RBC level(r=0.368), HCT level(r=0.409), RDW level(r=0.596), HMGB2 level(r=0.291) and IMT were positive (P<0.05, respectively). Multivariate logistic regression analysis indicated that RBC, HCT, RDW, and serum HMGB2 were independent risk factors for carotid atherosclerosis plaque formation(P<0.05, respectively). Conclusion: Elevated levels of RBC, HCT, RDW, serum HMGB2 have intimate connection with IMT, and have practical value in estimating the early atherosclerosis of hypertension patients from plateau area. |
