      摘要 目的:探讨彩色多普勒超声对糖尿病性大血管病变(DMD)血管狭窄程度和分布区域的诊断价值。方法:选择2015年1月到2017年1月在来院就诊的180例有2型糖尿病(T2DM)或大血管病变的患者作为研究对象,T2DM合并大血管病变者60例记为A组,大血管病变非T2DM者60例记为B组,单纯T2DM者60例记为C组。另选同期在医院接受健康体检的志愿者60例作为对照组,记为D组。检测对比A、B两组经彩超及血管造影检查的结果,分析影响DMD的单因素及危险因素。结果:A组的下肢动脉病变、血管狭窄检出率、严重狭窄检出率及≥3支血管病变检出率均较B组明显更高,而颈总动脉病变的检出率较B组明显更低,差异均有统计学意义(均P<0.05)。A、C两组的血糖(FPG)、糖化血红蛋白(HbA1c)、脂肪因子(Vaspin)及内脂素(Visfatin)水平均较D组明显更高,而A组以上指标又明显高于C组,差异均有统计学意义(均P<0.05)。A、C两组的甘油三酯(TG)、8-异前列腺素(8-iso-PGF2α)及内皮素(ET)水平较D组明显更高,且A组较C组明显更高,差异均有统计学意义(均P<0.05)。Logistic回归分析法分析显示影响DMD的危险因素有FBG、TG、8-iso-PGF2α及ET。结论:彩超对于DMD的诊断价值较好,此类病变的分布区域主要位于患者的下肢动脉,且影响DMD的危险因素有FBG、TG、8-iso-PGF2α及ET。

      ABSTRACT Objective: To discuss the diagnostic value of color doppler ultrasonography for the vascular stenosis degree and distri- bution of diabetic macrovascular disease. Methods: From January 2015 to January 2017, 180 cases of patients with type 2 diabetes melli- tus (T2DM) or macrovascular diseases were enrolled in our hospital for the study, 60 patients with T2DM complicated with macrovascu- lar diseases were recorded as group A; 60 patients with macrovascular disease and non T2DM were recorded as group B; 60 patients with simple T2DM were recorded as group C; over the same period, acceptance of healthy volunteers 60 cases in the hospital were control group, which was recorded as group D. The color Doppler ultrasound and angiography result were detected and compared between group A and group B, the single and risk factors of diabetic macrovascular disease were analyzed. Results: The detection rate of lower extremity arterial disease, vascular stenosis rate, severe stenosis rate and ≥3 vessel disease in group A was significantly lower than that of group B, and the common carotid artery lesion detection rate was significantly lower than group B(P<0.05). The blood glucose (FPG), glycosy- lated hemoglobin (HbA1c), fat factor (Vaspin) and visfatin (Visfatin) levels in group A, C were significantly higher than those in the group D , which were higher in the group A than those of group C(P<0.05). The levels of triglyceride (TG), 8-iso-PGF2α and endothelin (ET) in the group A, C were significantly higher than those in the group D, which were higher in the group A than those of the group C(P<0.05). According to Logistic regression analysis, the risk factors of diabetic macrovascular diseases was FBG, TG, 8-iso-PGF2α and ET. Conclusion: Color Doppler ultrasound had great value in the diagnosis of diabetic macrovascular diseases. The distribution of the le- sions mainly located in the lower extremities. And the risk of diabetic macrovascular disease factors were FBG, TG, 8-iso-PGF2α and ET.
