| 摘要 目的：探讨强直性脊柱炎（AS）患者外周血肿瘤坏死因子-α（TNF-α）、白细胞介素-17（IL-17）、白细胞介素-33（IL-33）的表达及与疾病活动程度的相关性。方法：选取2016年5月到2017年6月在我院接受治疗的AS患者74例作为研究组，另选取同期在我院体检的健康志愿者40例作为对照组，根据AS病情活动评分量表（BASDAI）将研究组患者分为A组（BASDAI>4分，32例）和B组（BASDAI得分≦4分，42例），比较研究组和对照组外周血中TNF-α、IL-17、IL-33水平，比较A组和B组外周血中TNF-α、IL-17、IL-33、C反应蛋白（CRP）、红细胞沉降率（ESR），采用Spearman相关性分析TNF-α、IL-17、IL-33水平与BASDAI得分及CRP、ESR的相关性。结果：研究组外周血中TNF-α、IL-17、IL-33水平明显高于对照组，差异有统计学意义（P<0.05），A组外周血中TNF-α、IL-17、IL-33、CRP、ESR明显高于B组，差异有统计学意义（P<0.05）；AS患者TNF-α、IL-17、IL-33水平与BASDAI得分及CRP、ESR水平均呈正相关（P<0.05）。结论：TNF-α、IL-17、IL-33在AS患者外周血中呈现高表达，并且与患者BASDAI得分及CRP、ESR水平呈正相关。 |

| ABSTRACT Objective: To investigate the expression of Tumor necrosis factor-α (TNF-α), interleukin 17(IL-17) and interleukin 33(IL-33) in the peripheral blood of patients with ankylosing spondylitis (AS) and their correlation with disease activity. Methods: A total of 74 patients with AS, who were treated in the Fifth Hospital of Xi'an from May 2016 to June 2017, were chosen as study group; another 40 healthy volunteers, who were examined in this hospital during the same period, were chosen as control group. The patients in the study group were further divided into group A (BASDAI score over 4 points, 32 cases) and group B (BASDAI score below or equal to 4 points,42 cases) according to the AS disease activity scale (BASDAI).The levels of TNF-α IL-17 and IL-33 in the peripheral blood of study group and control group were compared; the levels of TNF-α, IL-17, IL-33 , creactive protein (CRP) and erythrocyte sedimentation rate (ESR) in the peripheral blood of group A and group B were compared.The spearman correlation was used to analyze the correlation of TNF-α, IL-17 and IL-33 levels with BASDAI score, CRP and ESR. Results: The levels of TNF-α, IL-17 and IL-33 in the peripheral blood of the study group were significantly higher than those in the control group, the differences were statistically significant (P<0.05).The levels of TNF-α, IL-17, IL-33, CRP and ESR in the peripheral blood of group A were significantly higher than those in group B, the differences were statistically significant(P<0.05). The levels of TNF-α, IL-17 and IL-33 in AS patients were positively correlated with BASDAI scores, CRP and ESR levels(P<0.05). Conclusion: TNF-α, IL-17 and IL-33 are highly expressed in the peripheral blood of pa- tients with AS, and positively correlated with BASDAI score, CRP and ESR levels in patients with AS. |
