摘要 目的:本研究旨在调查糖尿病足的牙周病(PD)流行病学及其临床相关性。方法:选取2005年1月-2015年12月在本协作组医院就诊的4761例糖尿病患者作为研究对象,根据糖尿病足和牙周病的诊断标准筛查其中218例糖尿病足合并牙周病患者的临床资料。采用全自动生化分析仪检测患者血糖水平;采用放射免疫法检测患者血清总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)及高密度脂蛋白(HDL)水平;采用全自动生化分析仪检测患者糖化血红蛋白(HbAlc)含量;采用双抗体夹心酶联免疫吸附测定法检测患者血清25(OH)D水平;采用尿酸酶-过氧化物酶偶联法检测患者血尿酸(BUA)水平。结果:4761例糖尿病患者中,218例为糖尿病足伴牙周病(7.2%)。重度糖尿病足伴牙周病患者血糖、TC、TG、LDL、HDL、HbAlc及BUA水平均高于轻度糖尿病足伴牙周病患者和中度糖尿病足伴牙周病患者,差异均具有统计学意义(P<0.05);中度糖尿病足伴牙周病患者血糖、TC、TG、LDL、HDL、HbAlc及BUA水平均高于轻度糖尿病足伴牙周病患者,差异均具有统计学意义(P<0.05)。重度糖尿病足伴牙周病患者血清25(OH)D水平低于轻度糖尿病足伴牙周病患者和中度糖尿病足伴牙周病患者,差异具有统计学意义(P<0.05);中度糖尿病足伴牙周病患者血清25(OH)D水平低于轻度糖尿病足伴牙周病患者,差异具有统计学意义(P<0.05)。结论:血糖、血清总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)、糖化血红蛋白(HbAlc)、血清25(OH)D及血尿酸(BUA)水平与糖尿病足伴牙周病的发生及发展密切相关。 |

ABSTRACT Objective: To investigate the epidemiology and clinical relevance of periodontal disease(PD) in diabetic foot. Methods: 4761 diabetic patients were enrolled in this study from January 2005 to December 2015. According to the diagnostic criteria of diabetic foot and periodontal disease, 218 patients with diabetic foot and periodontal disease were screened clinical information. The blood glu- cose level was measured by automatic biochemical analyzer; The serum levels of total cholesterol (TC), triglyceride (TG), low density lipoprotein (LDL) and high density lipoprotein (HDL) were measured by radioimmunoassay. The serum level of HbAlc was measured by automatic biochemical analyzer; The serum level of 25 (OH) D was measured by double antibody sandwich enzyme-linked immunosor- bent assay (ELISA). The serum level of blood uric acid (BUA) was measured by uricase-peroxidase. Results: Among the 4761 patients with diabetes, 218 were diabetic foot periodontal disease (7.2%). The levels of blood glucose, TC, TG, LDL, HDL, HbAlc and BUA in patients with severe diabetic foot and periodontal disease were significantly higher than those of patients with mild diabetic foot and peri- odontal disease and moderate diabetic foot and periodontal disease, and the differences were statistically significant (P<0.05). The levels of blood glucose, TC, TG, LDL, HDL, HbAlc and BUA in patients with moderate diabetic foot and periodontal disease were significantly higher than those of the patients with mild diabetic foot and periodontal disease, and the differences were statistically significant (P<0.05). The levels of serum 25 (OH) D in patients with severe diabetic foot and periodontal disease were significantly lower than those of patients with mild diabetic foot and periodontal disease and moderate diabetic foot and periodontal disease, and the differences were sta- tistically significant (P<0.05); The levels of serum 25 (OH) D in patients with moderate diabetic foot and periodontal disease were signifi- cantly lower than those of the patients with mild diabetic foot and periodontal disease, and the differences were statistically significant (P<0.05). Conclusion: The blood glucose, total cholesterol (TC), triglyceride (TG), low density lipoprotein (LDL), high density lipoprotein (HDL), HbAlc, 25(OH) D and blood uric acid (BUA) are closely related to the occurrence and development of diabetic foot and peri- odontal disease. |
