      目的:分析血清同型半胱氨酸(Hcy)与胱抑素C(CysC)对早期2 型糖尿病肾病的临床诊断价值。方法:选择2013 年5 月至 2016 年5 月我院收治的早期糖尿病肾病患者60 例为A 组,选取同期我院收治的单纯糖尿病患者60例为B组,另选我院同期健 康体检者60 例为C 组,检测三组的血清Hcy、CysC、血尿素氮(BUN)、血肌酐(Scr)、尿蛋白排泄率(UAER)。结果:与C 组比较,A 组和B 组的BUN、Scr、UAER均升高(P<0.05),A 组的UAER 高于B 组(P<0.05),但A 组的BUN、Scr 与B 组比较,组间差异无 统计学意义(P 0.05)。A 组的病程明显高于B 组(P<0.05)。与C 组比较,A 组Hcy与CysC 水平升高(P<0.05),B 组的Hcy水平升 高(P<0.05),CysC 水平升高,但差异无统计学意义(P 0.05);与B 组比较,A 组Hcy 与CysC 水平升高(P<0.05)。A 组的Hcy、 CysC、Hcy+CysC 阳性率均明显高于B 组和C 组(P<0.05),B 组Hcy、CysC、Hcy+CysC 阳性率均明显高于C 组(P<0.05);A 组和 B 组的Hcy+CysC 联合检测的阳性率高于Hcy、CysC 的单独检测阳性率(P<0.05)。Hcy与病程、BUN、Scr、UAER均呈正相关,相 关系数r=0.650、0.488、0.596、0.761,P<0.05,CysC 与病程、BUN、Scr、UAER 均呈正相关,相关系数r= 0.681、0.502、0.601、0.825, P<0.05。结论:血清Hcy与CysC 可及时、准确的反应2型糖尿病患者的肾损伤情况,从而有利于早期2 型糖尿病肾病的及时检出。

      Objective:To investigate the clinical diagnostic value of Hcy and CysC in two-type diabetic nephropathy.Methods:60 cases of patients with two-type diabetic nephropathy in our hospital from May 2013 to May 2016 were selected as group A, 60 cases of patients with two-type diabetes were selected as group B, and 60 cases of physical examination were selected as group C. The levels of Hcy, CysC, BUN, Scr, UAER of three groups were compared.Results:The levels ofBUN, Scr, and UAER of group A and B were significantly higher than that of group C(P<0.05); the level of UAER and duration of group A were significantly higher than that of group B (P<0.05); the levels of Hcy, CysC of group A were significantly higher than that of group C(P<0.05); the levels of Hcy of group B were significantly higher than that of group C(P<0.05); the positive rate of Hcy, CysC, Hcy+CysC of was significantly higher in group A (P<0.05);the positive rate of combined detection of Hcy+CysC in group A and B were significantly higher than the positive rate of single detection of Hcy and CysC(P<0.05); both Hcy and CysC was positively correlated with the duration, BUN, Scr and UAER (P<0.05).Conclusion:The levels of Hcy and CysC can timely and accurately reflect renal damage in patients with type 2 diabetes, and contribute to the diagnosis of early two-type diabetic nephropathy.
