      目的:探讨超声引导下甲状腺穿刺活检(US-FNAB)对甲状腺良恶性结节的诊断价值。方法:本研究选取了60例已做过甲状 腺结节手术的患者,入院后先行常规超声(US)检查,后行超声引导下甲状腺穿刺活检(US-FNAB),通过观察并记录良恶性结节诊 断结果,诊断结果符合率及诊断结果符合率与结节大小的关系,评价US-FNAB对甲状腺良恶性结节的诊断价值。结果:常规超声 诊断出恶性结节4 例,良性结节33例;超声引导下细针穿刺活检诊断出恶性结节8 例,良性结节40例。US 组中,15 例误诊,8例 漏诊中,诊断符合率为61.7 %。US-FNAB组中,4 例误诊,漏诊2 例,诊断符合率80.0 %。经统计学分析,US-FNAB 组诊断符合率 明显高于US 组(P<0.05),US 组与US-FNAB组在各结节直径的患者分布上相比,差异无统计学意义(P>0.05)。两组组内各结节直 径的患者分布上相比,差异也无统计学意义(P>0.05)。US-FNAB 组中,随着结节直径增大,诊断符合率略有降低。对直径<10 mm 的甲状腺结节诊断率达100 %。结论:US-FNBA 对甲状腺良恶性结节具有较高的诊断准确性,诊断准确性受结节大小影响较小, 对直径较小的甲状腺结节检出率较高。

      Objective:To discuss the application value of ultrasound guided fine needle aspiration biopsy (US-FNAB) in the diagnosis of benign and malignant thyroid nodules.Methods:60 patients done with thyroid nodule surgery were selected, they were first given conventional ultrasound (US) examination and then US-FNAB. The application value of US-FNAB in the diagnosis of benign and malignant thyroid nodules was evaluated by the diagnostic results of benign and malignant nodule, coincidence rate and the relationship between the coincidence rate and size of the nodule.Results:There were 4 cases malignant nodules, 33 cases benign nodules diagnosed in the US group. There were 8 cases malignant nodules, 40 cases benign nodules diagnosed in the US-FNAB group. In the US group, 15 cases were misdiagnosed, 8 cases were missed diagnosis. The diagnostic coincidence rate was 61.7 %。In the US-FNAB group, 4 cases were misdiagnosed, 2 cases were missed diagnosis. The diagnostic coincidence rate was 80.0 %. After statistical analysis, the diagnostic coincidence rate of the US-FNAB group was higher than that of the US group (P<0.05). There were no statistical significance on diameters of the thyroid nodules between two groups (P>0.05). And there were no statistical significance on diameters of the in the group (P>0.05). In the US-FNAB group, the diagnostic accuracy was slightly decreased with the increase of diameter of nodules. Diagnosing the nodules <10 mm in diameter, the diagnostic coincidence rate was 100.0 %.Conclusion:US-FNBA has high diagnostic accuracy for benign and malignant thyroid nodules. The size of nodule has little influence on the diagnostic accuracy of US-FNBA, but the detection rate of thyroid nodules with the smaller diameter was high.
