| 目的：探讨乳腺化生性癌的病理特征、复发和生存情况。方法：选取我院于2002 年1 月至2015年12月期间收治的7 例乳腺
化生性癌患者临床病历资料进行回顾性分析，讨论乳腺化生性癌的病理特点和鉴别诊断、组织起源以及治疗预后等。结果：7 例乳
腺化生性癌PR、ER和HER-2 均显示为阴性，广谱CK则显示为阳性；所有入选患者均显示EGFR 阳性，其中有3 例患者显示p53
阳性，4 例患者显示CK5/6 阳性。此外，3 例患者经腋窝淋巴结清扫后显示有2 例患者出现腋窝淋巴结转移，4 例患者经前哨淋巴
结活检后结果显示无癌转移。结论：乳腺化生性癌在临床上属于乳腺癌中较为罕见的亚型，其生物学行为和生存预后均不佳，
HER-2 和PR、ER等多显示为阴性，容易复发，目前主要治疗方法为在手术治疗的基础上加以放射治疗。 |

| Objective:To investigate the pathological characteristics, recurrence and survival of breast metaplastic cancer.Methods:The clinical data of 7 cases of breast metaplastic carcinoma in our hospital during January 2002 and December 2015were
retrospectively analyzed. The differential diagnosis and pathological features, histogenesis, treatment and prognosis of breast metaplastic
carcinoma were discussed.Results:In the 7 cases of metaplastic carcinoma of breast, the PR, ER and HER-2 were negative, whereas the
broad spectrum CK showed positive. All enrolled patients showed positive EGFR, including 3 cases of patients showing positive p53 and
4 cases showing positive CK5/6. In addition, 3 cases had axillary lymph node dissection, and 2 of them had axillary lymph node
metastasis.After sentinel lymph node biopsy, 4 patients showed no metastasis.Conclusion:Breast metaplastic carcinoma clinically
belonged to the rare subtypes of breast cancer. Its biological behavior and prognosis were poor. The PR and HER-2, ER and other
indicators displayed negative. It was easy to relapse. Presently the main treatment for breast metaplastic carcinoma was surgery plus
radiation therapy. |
