| 目的：比较分析关节镜下不同术式治疗盘状半月板的临床效果。方法：选取盘状半月板患者85 例，其中完全型盘状半月板
47 例，行完全切除术(A 组)23 例，行部分切除成形术(B 组)24 例；不完全性盘状半月板38 例，行完全切除术(C 组)20 例，行部分切
除成形术(D 组)18 例。术后随访观察，膝关节功能评价分别采用国际膝关节文献委员会膝关节评估表(the international knee documentation
committee knee evaluation form，IKDC)和Lysholm 评分进行。结果：78 例患者均得到有效随访。随访时间12-18 个月，平
均(16.2± 3.4)月。术前各组各评分均无显著差异(P>0.05)。完全型盘状半月板中，A组术后IKDC 和Lysholm评分分别为(92.9±
9.7)分、(93.4± 8.6)分，优良率为91.3%(21/23)，均显著优于B 组[(83.5± 8.4)分、(82.7± 9.2)分、70.8%(17/24)](P<0.05)。C 组和D 组
术后各指标比较无显著差异(P>0.05)。结论：关节镜下不同术式治疗盘状半月板均可获得较为满意的疗效，但对于完全型盘状半
月板而言，采取完全切除术治疗的患者较部分切除成形术者术后疗效具有一定优势。 |

| Objective:To explore the clinical outcome of arthroscopic treatment of discoid meniscus injuries of the knee joints in
different operative methods.Methods:85 patients with discoid meniscus were selected. 47 cases with complete type of discoid meniscus
were divided into 2 groups. GroupA (23cases) were treated by total meniscectomy and groupB (24cases) were treated by partial meniscectomy.
The rest of 38 cases with incomplete type of discoid meniscus were divided into 2 groups. Group C(20cases) were treated by total
meniscectomy, and groupD (18cases) were treated by partial meniscectomy. Postoperative follow-up assessment included the IKDC
and Lysholmscore.Results:85 patients were effectively followed up for 12-18 months. The average month is (16.2± 3.4). There was no
sig- nificant difference in the scores of each group before surgery (P>0.05). Complete type of discoid meniscus, a group of postoperative
IKDC and Lysholm score were (92.9± 9.7) and (93.4± 8.6) respectively. The excellent and good rate was 91.3% (21/23). It was significantly
better than B group (83.5± 8.4)points,(8.27± 9.2)points,70.8%(17/24)] (P<0.05). There was no significant difference between
group C and group D after surgery (P>0.05).Conclusion:Different surgical treatments of discoid meniscus injuries can obtain curative
effect satisfactorily. But to the complete type of meniscus patients, the patients who treated with complete resection have some advantages
compared with the effect of partial resection of the patients. |
