| 目的：观察和比较腹腔镜与开放手术治疗复杂性阑尾炎的临床效果及安全性。方法：选取2012 年4 月～2014 年5 月于我院
(总院及分院)诊治的复杂性阑尾炎患者共131例，按照随机原则分为对照组65 例和观察组66 例，对照组实施开腹阑尾切除术治
疗，观察组实施腹腔镜阑尾切除术治疗。对比两组患者各项术中、术后临床指标。结果：两组的手术时间比较无明显差异(P＞0.05)；
观察组患者的手术切口长度、术中出血量、术后肠功能恢复时间、置管引流时间、术后24 hVAS、住院时间均明显优于对照组，差
异具有统计学意义( P＜0.05)；观察组(12.1 %)并发症总发生率低于对照组(27.7 %)( P＜0.05)。两组肠梗阻发生率相近，无明显差异
(P＞0.05)。结论：腹腔镜手术治疗复杂性阑尾炎较开腹阑尾切除术具有创伤小、出血少、疼痛轻、住院时间短、恢复快的优势，且安
全性更高。 |

| Objective:To observe and compare the clinical effect and safety of laparoscopic and open operation in the treatment of
complicated appendicitis.Methods:132 cases of patients with complicated appendicitis admitted in our hospital (institute and branch)
from April 2012 to May 2014 were chosen and randomly divided into the control group (65cases) and the observation group (66 cases).
The control group received open appendectomy treatment, while the observation group was given laparoscopic appendectomy treatment.
The clinical indexes of two groups in the operation and postoperation were compared.Results:There was no obvious difference between
the two groups in the operation time (P>0.05). The operation length of incision, bleeding volume, postoperative intestinal function recovery
time, time of drainage, VAS at 24 h postoperation and time of hospitalization of observation group were significantly better than those
of the control group(P<0.05). The total incidence rate of complications of observation group (12.1 %) was significantly lower than that of
the control group (27.7 %) (P<0.05), but no significant difference was found in the incidence rate of intestinal obstruction between two
groups(P>0.05).Conclusion:Compared with open operation, laparoscopic appendectomy had advantages of less wound, bleeding, postoperative
pain and shorter hospital stay as well as high safety in the treatment of complicated appendicitis. |
