目的:探讨儿童阻塞性睡眠呼吸暂停/低通气综合征(OSAHS)与肥胖的相关性。方法:收集单纯性肥胖儿童120 例和体重正
常儿童110 例作为研究对象,进行统一的体格检查和专科检查,并进行多导睡眠监测记录阻塞性呼吸暂停指数(OAI)、呼吸暂停/
低通气指数(AHI)、中枢性呼吸暂停指数(CAI)、最低血氧饱和度(LSaO2)和睡眠效率。结果:肥胖组OSAHS 患病率为58.33%显
著高于对照组的31.82%,差异有统计学意义(P<0.05);OAI、AHI、CAI 均显著高于对照组,而LSaO2、睡眠效率指标显著低于对照
组,差异均有统计学意义(均P<0.05);多因素回归分析显示,肥胖、扁桃体增生、腺样体增生是导致OSAHS 的独立危险因素,差异
有统计学意义(均P<0.05)。结论:肥胖是儿童OSAHS 发病的重要影响因素,特别是合并扁桃体肿大或腺样体肿大的患儿应注意
预防OSAHS 的发生。 |

Objective:To explore the correlation of obstructive sleep apnea apnea/hypopnea syndrome (OSAHS) and obesity in
children.Methods:120 cases of obesity children and 110 cases of normal weight children were enrolled as object, every children were
performed with physical examination,specialist examination and sleep monitoring, recorded the obstructive apnea index (OAI),
apnea/hypopnea index (AHI), center apnea index (CAI), lowest saturation of oxygen (LSaO2) and sleep efficiency by polysomnography
monitoring.Results:The OSAHS prevalence rate in obesity group was 58.33%, which was significantly higher than 31.82% of the
control group, the difference was statistically significant (P<0.05). The indexes of OAI, AHI and CAI in obesity group were significantly
higher than those in control group (P<0.05), while the LSaO2 and sleep efficiency in obesity group were significantly lower than those in
control group (P<0.05). Logistic regression analysis showed that obesity, antiadoncus and adenoids were the risk factor of OSAHS(P<0.
05).Conclusion:Obesity is an important factor in the pathogenesis of OSAHS in children,for obese children especially the tonsils,
adenoids in children should be alert to the occurrence of OSAHS. |
