| 目的：研究比较三种不同手术方案治疗急性闭角型青光眼（AACG）合并白内障患者的疗效。方法：选择2012 年10 月至
2015 年10 月在我院接受手术治疗的AACG 合并白内障患者50 例（50 只眼）进行研究，按患者前房角的粘连关闭程度将其分成
A、B、C三组，A组给予小切口囊外摘除术，B组在A组基础上给予前房角分离术，C 组在A 组基础上给予小梁切除术，比较各组
术后疗效。结果：A、B、C三组患者术后的眼压均显著低于术前，前房中央深度和视力均显著高于术前，差异均有统计学意义（P＜
0.05）。A、B、C 三组术后视力的分布中，0.3～0.5 的比例均显著高于＜0.3 及＞0.5 的比例，差异有统计学意义（P＜ 0.05）。A 组术后
的并发症总发生率为15.79%，B组为31.25%，C组为40.00%，三组间比较差异无统计学意义（P＞ 0.05）。结论：AACG 合并白内障
患者可通过多种手术方案进行治疗，但应根据房角关闭程度合理地选择最佳术式，从而有效改善患者的症状和视力状况。 |

| Objective:To compare the efficacy of three different surgical procedures in the treatment of patients with acute angle
closure glaucoma (AACG)combined with cataract.Methods:Selected 50 cases of patients (50 eyes)with AACG combined with cataract
who underwent surgical treatment in our hospital from October 2012 to October 2015 as object, and were divided into A, B, C groups according
to the degree of closure of anterior chamber angle in patients,A group was treated with small incision extra capsular extraction, B
group was treated with the anterior chamber angle separation on the basis ofA group, C group was treated with trabeculectomy basis of A
group, compared the efficacy of each groups after operation.Results:The intraocular pressure of patients in A, B, C groups after operation
were significantly lower than before operation, the central anterior chamber depth and visual acuity were significantly higher than before
operation, the differences were statistically significant (P<0.05). In the distribution of postoperative visual acuity in A, B, C groups, the
proportion of 0.3 to 0.5 was significantly higher than that <0.3, and >0.5, the differences were statistically significant (P<0.05). The total
incidence of postoperative complications in A group was 15.79%, B group was 31.25%, C group was 40%, there were no significant difference
between the three groups (P>0.05).Conclusion:AACG combined with cataract patients can be treated with a variety of surgical
procedures, but should be based on the degree of closure of anterior chamber angle to choose the best method, so as to effectively improve
the symptoms and visual acuity of patients. |
