目的:研究64 排螺旋CT 不同重建方法在桡骨下端隐匿性骨折(OFDR)诊断中的临床应用。方法:选择2012 年6 月至2015
年6 月在我院接受手术治疗的46 例OFDR 患者作为研究对象,所有患者均通过64 层的Somatom Definition AS 型CT 机实施双
侧腕关节的容积扫描,分别利用轴位、矢状位及冠状位MPR以及VR 等重建方法评价OFDR 图像,对比不同方位的多平面重建
(MPR)图像与容积重建(VR)图像对于OFDR的显示率情况,CT 不同重建方法的诊断情况及诊断价值。结果:四种方式联合检测
对于OFDR的显示率最高,为100.00%。矢状位MPR 及冠状位MPR 对于OFDR 的显示率均分别显著高于轴位MPR 及VR,差
异均有统计学意义(P<0.05)。轴位MPR 的真阳性26 例,真阴性4 例;矢状位MPR 的真阳性41 例,真阴性1例;冠状位MPR 的
真阳性36 例,真阴性2 例;VR的真阳性23例,真阴性1 例;四种方式联合检测的真阳性44 例,真阴性2 例。四种方式联合检测
的灵敏度、特异度及准确度均最高,为100.00%,其次为矢状位MPR及冠状位MPR。结论:64 排螺旋CT 能够较好地诊断OFDR,
重建方法应用矢状位MPR 及冠状位MPR可获得更为清晰的图像显示价值,效果明显,值得临床应用。 |

Objective:To study clinical application of 64 slice spiral CT in the diagnosis of occult fractures of the distal radius
(OFDR).Methods:A total of 46 patients with OFDR, who were operated on in Wuhan Hankou Hospital of Hubei Province from June
2012 to June 2015, were chosen as the study subjects. All the bilateral wrist joints of patients were scanned by 64 slice CT(type:
SOMATOMDefinition AS). OFDR images were respectively evaluated by using axial, sagittal and coronal MPR and VR reconstruction
methods. OFDR display rates were compared between different orientations of multi planar reconstruction (MPR) images and volume
reconstruction (VR) images. The diagnosis and diagnostic value of CT with different reconstruction methods were compared.Results:The rate of OFDR was 100% by the combining detection of the four methods. The display rates of sagittal MPR and coronal MPR were
significantly higher than those of axial MPR and VR, the differences were statistically significant (P<0.05). There were 44 cases of true
positive and 2 cases of true negative in the four ways of combined detection, and their sensitivity, specificity and accuracy are the highest
(100%), followed by sagittal MPR and coronal MPR.Conclusion:64 slice spiral CT can be used to diagnose OFDR, and the
reconstruction method of MPR and coronal MPR can obtain better image display and effects,which is worthy of clinical application. |
