      目的:探讨螺旋CT 扫描及三维重建技术在股骨颈骨折分型及治疗中的应用价值。方法:选择2010 年5 月~2013 年5 月期 间我院收治的股骨颈骨折患者237 例为研究对象,根据患者扫描检查方式的不同将其分为对照组(112 例)和观察组(125 例),对 照组患者行髋关节X 线正位扫描,观察组行髋关节正位64 排螺旋CT扫描,两组均根据扫描结果进行分型并制定相应的治疗方 案,比较两组患者骨折内固定手术后2 年的股骨头坏死率及骨折不愈合率。结果:两组行骨折内固定手术比例比较,差异无统计 学意义(P>0.05);术后2 年,对照组股骨头坏死5 例(22.73%),骨不连6 例(27.27%);观察组股骨头坏死1 例(3.70%),骨不连1 例(3.70%),观察组患者股骨头坏死率及骨折不愈合率均显著低于对照组,差异均有统计学意义(P<0.05)。结论:螺旋CT扫描及 三维重建成像能够全面、准确显示股骨颈骨折的损伤情况,有助于骨折的正确分型和治疗方法的选择,改善预后。

      Objective:To explore the clinical value of spiral CT scanning and 3D reconstruction technology in classification and treatment of femoral neck fracture.Methods:A total of 237 patients with femoral neck fracture, who were admitted to Shenyang Orthopedic Hospital from May 2010 to May 2013, were selected and divided into control group (n=112) and observation group(n=125) according to the scanning method. The control group underwent hip joint X-ray scanning,while the observation group underwent 64 slice spiral CT scanning. The patients of two group were classified and formulated corresponding treatment according to the scanning results. Then the femoral head necrosis rate and fracture nonunion rate 2 years after the fracture internal fixation operation of the two groups were compared.Results:There was no statistical difference in the proportion of fracture internal fixation operation between the two groups (P>0.05). 2 years after the fracture internal fixation operation, there were 5 patients with femoral head necrosis (22.73%) and 6 patients with bone nonunion (27.27%) in the control group; there was 1 patient with femoral head necrosis (3.70%) and 1 patient with bone nonunion (3.70%) in the observation group. The femoral head necrosis rate and fracture nonunion rate in the observation group were significantly lower than those in the control group, the differences were statistically significant (P<0.05).Conclusion:Spiral CT scanning and 3D reconstruction can comprehensively and accurately display the injury state of femoral neck fracture and improve the prognosis, which is helpful to the correct classification and selection of treatment.
