      目的:探讨血清胱抑素C(Cys-C)和视黄醇结合蛋白(RBP)水平联合检测在早期糖尿病肾病(DN)诊断中的应用价值。方法: 选择2010 年9 月到2014 年9 月在我院诊疗的150 例DN患者,依照24 h尿清蛋白排泄率(UAER)分为DM 组(n=60 例)、早期 DN组(n=46 例)和临床DN组(n=44例),同期选择82 例健康体检者作为对照组。检测所有患者血清Cys-C、RBP、尿素氮(BUN) 和肌酐(Cr)的浓度,比较各指标的阳性检测率和早期DN组各指标单一、联合检测的灵敏度。结果:早期DN 组和临床DN组血清 Cys-C、RBP 浓度明显高于DM 组和对照组(P<0.05);临床DN 组血清Cys-C、RBP 浓度明显高于早期DN 组(P<0.05)。早期DN 组血清BUN、Cr 浓度与DM 组和对照组相比,无显著性差异(P>0.05);临床DN组血清BUN、Cr 浓度明显高于DM 组和对照组 (P<0.05)。早期DN组和临床DN 组血清Cys-C、RBP、BUN及Cr阳性检出率比较无显著性差异(P>0.05);早期DN 组和临床DN 组血清Cys-C、RBP 阳性检出率明显高于血清BUN、Cr(P<0.05)。早期DN 组血清Cys-C+RBP联合检测灵敏度明显高于血清 Cys-C、RBP、BUN、Cr 的单一检测灵敏度和血清BUN+ Cr 联合检测灵敏度(P<0.05)。结论:血清Cys-C 和RBP 水平联合检测较 BUN、Cr检测早期DN,阳性检出率和灵敏度高,值得在临床上推广应用。

      Objective:To investigate the application value of serum Cystatin C (Cys-C) combined with retinol binding protein (RBP) detection in diagnosis of early diabetic nephropathy (DN).Methods:A total of 150 patients with early DN, who were treated in Tianjin 254 Hospital fromSeptember 2010 to September 2014, were selected and divided into diabetes mellitus(DM) group(n=60), early DN group(n=46) and clinical DN group(n=44) according to the level of UAER within 24 hours; 82 healthy people were chosen as control group. The concentration of serum Cys-C, RBP, BUN and Cr of all the subjects were detected; The positive detection rates of all the indexes of the four groups and the sensitivity of single and combined indexes in the early DN group were compared.Results:The concentration of serum Cys-C and RBP in the early DN group and clinical DN group were significantly higher than that in the DM group and control group (P<0.05); The concentration of serum Cys-C and RBP in the clinical DN group were significantly higher than that in the early DN group(P<0.05). The concentration of serumBUN and Cr in the early DN group had no significant difference compared with the DM group and control group (P>0.05). But the concentration of serum BUN and Cr in the clinical DN group were significantly higher than that in the DM group and control group (P<0.05). There were no significant differences in the positive detection rates of serum Cys-C, RBP, BUN and Cr between the early DN group and the clinical DN group (P>0.05). The positive detection rates of serum Cys-C and RBP were significantly higher than those of serumBUNand Cr in the early DNgroup and clinical DNgroup (P<0.05). The sensitivity of serum Cys-C combined with RBP detection was significantly higher than that of single serum Cys C, RBP, BUN and Cr detection and serumBUN combined with Cr detection in the early DN group(P<0.05).Conclusion:The serumCys-C combined with RBP detection has a higher positive detection rate and sensitivity in the diagnosis of early DN, which is worthy of clinical application.
