目的:研究超低剂量扫描在CT 引导下肺部穿刺活检中的应用。方法:2009年1 月至2014 年12 月来本院200例行胸部CT
引导下穿刺的患者分为常规剂量组(130 kV,90 mAs)、超低剂量组(100 kV,7.5 mAs),各组100 例,并进行穿刺。根据CT 机自动记
录的受检者剂量长度乘积(dose length product, DLP)计算X 线有效辐射剂量(effective dose, ED)。比较两组在辐射剂量、图片质
量、穿刺成功率的差异。结果:超低剂量组和常规剂量组DLP 分别为(25.79 ± 1.13) mGy·cm 和(256.14 ± 4.48) mGy·cm(P<
0.05),ED 分别为(0.36 ± 0.02) mSV 和(3.59 ± 0.06) mSv(P<0.05)。图片质量均为A级。超低剂量组和常规剂量组穿刺成功率分别
为88 %(88/100)为92 %(92/100),差异无统计学意义(x2=0.89, P>0.05)。结论:超低剂量CT 引导下胸部穿刺,可降低受检者的X
线有效辐射剂量,图像质量可达到临床CT 引导下胸部穿刺的要求,值得提倡和推广。 |

Objective:To study application of low dose CT-guided puncture biopsy of lung.Methods:From January 2009 to December
2014, 200 consecutive patients prepared for lung biopsy were divided into standard dose group and low-dose group. Of these patients,
100 patients underwent percutaneous needle lung biopsy under standard dose CT scan (130 kV, 90 mAs), while the other 100 patients
underwent low-dose CT scan (100 kV, 7.5 mAs) and puncture. The effective dose (ED) was calculated by dose length product
(DLP) automatically recorded by the CT machine. And the statistical analyses on ED and DLP from two groups were performed. Data
from two groups were analyzed for radiation dose, image quality and puncture success rate.Results:The mean DLP and ED were
(25.79± 1.13) mGy·cm and (0.36± 0.02) mSv in low-dose group and (256.14± 4.48) mGy·cm and(3.59± 0.06) mSv in the standard-
dose group respectively, both parameters indicating significant difference between two groups respectively (P<0.05). The image
quality of the two groups were both level A. Also, there was no statistical difference on success puncture rate (x2=0.89, P>0.05) in
low-dose group (88 %, 88/100) compared to the standard-dose group (92 %, 92/100).Conclusion:Low-dose chest CT-guided biopsy can
significantly reduce the radiation dose, while the image quality meet requirements of clinical chest CT-guided biopsy. |
