      目的:探讨异丙托溴铵联合布地奈德对慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭患者MDA、SOD、GSH-Px 水平及肺功能的影 响。方法:选取我院收治的慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭患者92 例,分为对照组和实验组,每组各46 例。对照组采用常规 治疗,实验组在对照组基础上加用异丙托溴铵溶液联合布地奈德雾化吸入治疗。观察并比较两组患者治疗前后MDA、SOD 及 GSH-Px 水平的变化情况以及肺功能和血气指标的改善情况。结果:与治疗前比较,两组患者治疗后SOD、GSH-Px、FEV1、VC、 PaO2明显升高,而MDA、RV、TLC、PaCO2明显降低(P<0.05);与对照组比较,实验组治疗后SOD、GSH-Px、FEV1、VC、PaO2明显 升高,而MDA、RV、TLC、PaCO2明显降低(P<0.05)。结论:异丙托溴铵联合布地奈德能够改善慢性阻塞性肺疾病合并Ⅱ型呼吸衰 竭患者的肺功能参数、血气指标和MDA、SOD、GSH-Px 水平,增强机体抗氧化功能,提高临床疗效。

      Objective:To investigate the effect of ipratropium bromide combined with budesonide on the changes of MDA, SOD and GSH-Px levels and pulmonary function of patients with chronic obstructive pulmonary emphysema and type II respiratory failure.Methods:92 patients with chronic obstructive pulmonary emphysema and type II respiratory failure were selected and randomly divided into the control group and the experiment group, with 46 cases in each group. The patients in the control group were treated with the conventional method, while the patients in the experiment group were treated with the ipratropium bromide and budesonide inhalation besides the conventional method. Then the levels of SOD, GSH-Px and MDA, the pulmonary function and blood gas indexes were observed and compared before and after the treatment.Results:After the treatment, the levels of SOD, GSH-Px, FEV1, VC and PaO2 in the two groups were higher than before, and the levels of MDA, RV, TLC and PaCO2 were lower with statistically significant differences (P<0.05). Compared with the control group, the levels of SOD, GSH-Px, FEV1, VC and PaO2 in the experiment group were higher, and the levels of MDA, RV, TLC and PaCO2 were lower with statistically significant differences (P<0.05).Conclusion:The ipratropium bromide and budesonide can improve the pulmonary function parameters, blood gas indexes and MDA, SOD and GSH PX levels of patients with COPD and type II respiratory failure, and to strengthen the function of antioxidant system.
