      目的:探究细胞因子的表达与抗体反应在子宫内膜异位症(EMS)发生中的作用及其作为诊断指标的可能性。方法:选取子宫 内膜异位症患者70 例与对照组50 例,检测并比较两组患者血清肿瘤坏死因子-alpha(TNF-alpha)、白细胞介素(IL-6)、单核细胞趋化因子 -1(MCP-1)、血管内皮生长因子(VEGF)的表达以及抗子宫内膜抗体(EM-Ab)和转铁蛋白抗体(TRF-Ab)的阳性率。分析以上细胞因 子及抗体的表达与子宫内膜异位症临床分期的关系。结果:子宫内膜异位症组TNF-琢、IL-6、MCP-1 与VEGF均显著高于对照组, 差异具有统计学意义(P<0.05);子宫内膜异位症组抗子宫内膜抗体阳性率、转铁蛋白抗体阳性率以及两种抗体均为阳性的比例均 显著高于对照组,差异具有统计学意义(P<0.05);TNF-琢、IL-6、MCP-1 与VEGF细胞因子与子宫内膜异位症分期呈正相关,分期 越高、EMS病情越重则细胞因子表达水平越高(P<0.05)。结论:细胞因子的表达与相关抗体反应均参与了子宫内膜异位症的发生 及发展,通过检测到相关细胞因子水平的升高与自身抗体转阳可以筛选与诊断子宫内膜异位症,初步判断其临床分期和病情程 度。

      Objective:To explore the expression of cytokines and antibody response in the endometriosis (EMS) patients and its possibility as the diagnostic indexes of this disease. Methods: 70 cases with endometriosis and 50 cases of non- endometriosis (control group) were selected as research objects. using enzyme-linked immunoassay detection, the serum tumor necrosis factor alpha (TNF-alpha), interleukin (IL-6), monocyte chemotactic factor 1 (MCP 1) and vascular endothelial growth factor (VEGF) and resistance of endometrial antibody (EM-Ab), transferrin antibody (agency-Ab) was detected and compared between two groups. The correlation between the cytokines and the staging and severity of endometriosis was statistically analyzed.Results:The expressions of TNF-琢, IL - 6, MCP - 1 and VEGF in the serum of patients with endometriosis were significantly higher than those of the control group with statistically significant differences (P<0.05). In the patients with endometriosis, the positive rates of endometrial antibody and transferrin antibody and the proportion that both of the two antibodies were significantly higher than those of the control group with statistically significant differences (P<0.05). The levels of TNF-琢, IL - 6, VEGF and MCP - 1 were positively correlated with the staging of endometriosis (P<0. 05), the higher stage, more severe EMS, then the level of cytokines was higher.Conclusion:The expressions of cytokines and antibody response are involved in the formation and development of the EMS, and the detection of the level of the related cytokines and the changes of autoantibodies can contribute to screening and diagnosis for endometriosis, making the preliminary judgment for the clinical stage and severity of illness.
