| 目的：探讨前外侧与后外侧路小切口髋关节置换术治疗老年股骨颈骨折临床疗效。方法：选取我院创伤骨科老年股骨颈骨折
患者92 例，随机分为两组，A 组46 例，予以后外侧路小切口髋关节置换术治疗；B 组46 例，予以前外侧小切口髋关节置换术治
疗。记录并比较两组患者手术临床资料、术后并发症情况，采用Harris 评分评价术后关节功能，调查问卷调查术后满意度。结果：①
A 组患者手术切口长度、手术时间、术中出血量和术后引流量明显小于B 组患者，差异有统计学意义（P＜0.05）；然两组患者住院
时间相比无明显差异（P＞0.05）；② A组术后关节功能与B组相比较，无明显差异（P＞0.05）；而A组术后并发症发生率明显低于
B组，差异有统计学意义（P＜0.05）；③ A 组的患者术后满意率明显高于B 组，差异有统计学意义（P＜0.05）。结论：前外侧和后外
侧路小切口髋关节置换术治疗老年股骨颈骨折临床疗效相似，但后外侧路小切口操作简便，能够缩短手术时间、减少手术出血
量、降低术后并发生症发生，患者满意度较高，对老年股骨颈骨折治疗临床价值较高。 |

| Objective:To investigate the application of small incision hip replacement with different approach in elderly femoral
neck fracture.Methods:92 elderly patients with femoral neck fractures were selected and divided into group A and group B. 46 elderly
patients with femoral neck fractures in group A were treated with posterolateral approach small incision for treatment of hip replacement .
46 elderly patients with femoral neck fracture in group B were treated with anterolateral small incision hip replacement. The clinical data
and postoperative complications were recorded. Postoperative joint function was compared by Harris score evaluation.Results:Compared with the group B, the length of the surgical incision, the operation time, perioperative bleeding volume and postoperative
drainage fluid was lower in the group A with statistical significance （P ＜0.05）. Compared with the group B, the incidence of
complications were lower in group A（P＜0.05）. Compared with the group B, the satisfaction rate in the patients of group A was higher
（P ＜0.05）. There was no difference in the hospitalization day and postoperative joint function between two groups (P>0.05).Conclusion:Posterolateral approach small incision hip replacement can shorten operation time, reduce the amount of bleeding and
reduce postoperative complications with high satisfaction of patients in the treatment of femoral neck fracture in the elderly. |
