| 目的：观察水蛭素联合改良三仁活血降脂方治疗肾病综合征湿浊血瘀证患者的临床疗效。方法：选取肾病综合征辨证属湿浊
血瘀证患者60 例随机分为水蛭素联合中药治疗组和单纯中药对照组,每组各30 例。两组患者均予以相同的基础治疗,对照组予
改良三仁活血降脂方辨证论治,治疗组在此基础上加用脉血康胶囊，疗程半年。比较两组患者中医临床症候积分，血凝、血脂、血浆
白蛋白、尿蛋白及肾功能等指标。结果：治疗后两组患者湿浊血瘀证候均有改善，其中治疗组血瘀证候改善明显优于对照组（P<0.
05）。治疗后两组患者血浆白蛋白水平均较治疗前明显上升（P<0.01）；治疗组纤维蛋白原、胆固醇、24 h尿蛋白定量治疗效果优于
对照组（P<0.01）；治疗组治疗后甘油三酯下降，血肌酐下降，差异有统计学意义（P<0.05），对照组二者均无明显效果（P>0.05）。治
疗组与对照组的中医证候总有效率分别为96.67 %、90 %，西医临床疗效分别为90 %、86.67 %。结论：水蛭素能有效改善肾综湿浊
血瘀证患者的中医临床证候及实验室指标，部分调节肾病综合征的高凝、高脂状态。 |

| Objective:To observe the clinical effect of hirudin combined with Modified Sanren Huoxue Jiangzhi decoction in the
treatment of nephrotic syndrome patients with blood silt and damp pathogen syndrome.Methods:60 cases of nephritic syndrome patients
with wet turbid and blood stasis were randomly divided into hirudin combined with traditional Chinese medicine treatment group and
pure Chinese medicine control group, 30 cases in each group. Two groups of patients were given the same basic treatment, the control
group was given the Modified Sanren Huoxue Jiangzhi decoction according to syndrome differentiation , the treatment group based on
the use of Maixuekang capsule, Six months for a course of treatment. TCM symptoms integral, blood coagulation, blood lipids, plasma
albumin, urinary protein and renal function indices of two groups were compared.Results:After the treatment, two groups of patients
with blood stasis syndrome of dampness were improved, in the treatment group, blood stasis syndrome is significantly better than the
control group, (P<0.05,). Compared with before treatment，patients with plasma albumin levels in the two groups were significantly
increased (P<0.01); The therapeutic effects of the treatment group were better than that of the control group in fibrinogen, cholesterol and
the quantity of the urinary protein in 24 hours (P<0.01); treatment group decreased triglycerides, and serum creatinine, difference has
statistical significance (P<0.05), that in the control group were no significant effect (P>0.05). TCM treatment group and control group in
the total effective rate was 96.67 %, 90 % respectively, the clinical effect of Western medicine respectively 90 %, 86.67 %.Conclusion:Hirudin can effectively improve the TCMsyndromes and laboratory indexes in patients with blood silt and damp pathogen syndrome, and
partly regulate the hypercoagulable and hyperlipidemic state of nephrotic syndrome. |
