      目的:探讨右美托咪定对慢性阻塞性肺疾病急性加重期(AECOPD)患者机械通气的镇静效果和安全性。方法:选取2012 年6 月-2014 年9 月在我院接受气管插管呼吸机辅助通气治疗的AECOPD患者62 例,并将其随机分为实验组和对照组。其中,对照 组患者给予常规治疗,实验组在常规治疗的基础上给予右美托咪镇痛。观察和比较两组患者机械通气持续时间、ICU停住时间、呼 吸机相关性肺炎(VAP)、心动过缓和谵妄的发生率。结果:实验组患者机械通气时间及ICU 停住时间均明显短于对照组,VAP、心 动过缓及谵妄的发生率均显著低于对照组,差异均具有统计学意义(P<0.05)。结论:右美托咪定对行机械通气慢性阻塞性肺疾病 急性加重期患者良好的镇静效果,且安全性高,值得临床推广。

      Objective:To explore the sedative effect and safety of dexmedetomidine for patients on the acute aggravating chronic obstructive pulmonary disease (AECOPD) under mechanical ventilation.Methods:62 patients with tracheal intubation respirator assisted ventilation, who were treated in our hospital from June 2012 to September 2014, were selected and randomly divided into the experimental group and the control group. The patients in the control group were treated with the conventional method, while the patients in the experimental group were treated with dexmedetomidine besides of the conventional treatment. Then the time of mechanical ventilation and the incidence of the ventilator associated pneumonia (VAP) and deliriumof patients in the two groups were observed and compared.Results:The mechanical ventilation time and ICU stay of patients in the experimental group were shorter than those of the control group with statistically significant differences (P<0.05); the incidences of VAP, bradycardia and delirium of patients in the experimental group were lower than those of the control group with statistically significant differences (P<0.05).Conclusion:Dexmedetomidine had better sedative effect and higher safety for the patients with AECOPD under mechanical ventilation.
