      目的:探讨我院产超广谱β-内酰胺酶(ESBLs) 肺炎克雷伯菌阳性率及耐药性的变化,以指导临床合理应用抗生素。方法: 对我院自2009 年1 月至2011 年8 月住院患儿送检的各类标本中分离得到的肺炎克雷伯菌413 株进行ESBLs 筛选和确证。结 果:共分离得到413 株肺炎克雷伯菌,其中产ESBLs 的菌株共204 株,占49.39%,且有逐年上升趋势。ESBLs 阳性菌株对头孢他 啶、头孢噻吩、头孢噻肟、阿莫西林耐药程度最为严重,耐药率为100%;对美洛培南、亚胺培南、哌拉西林/ 他唑巴坦、阿米卡星、环 丙沙星高度敏感,敏感率为90.2%~100%。结论:产ESBLs 的肺炎克雷伯菌已成为主要耐药菌,应加强耐药监测,重点监管并指导 临床合理应用抗生素。

      Objective: To investigate the infection rate and drug resistance status of extended-spectrum β-lactamases producing (ESBLs) Klebsiella pneumoniae and to provide the guidance for clinic anti-infective treatment. Methods: The ESBLs were analyzed in 413 strains of Klebsiella pneumoniae isolated from our neonatal ward from Jan. 2009 to Aug. 2010. All the analyses were conducted according to the code of CLSI M100-S19. test. Results: Among the identified 413 strains of Klebsiella pneumoniae, 49.39%of them(204 strains)produced ESBLs,and the numbers showed ascend trend year by year. The drug resistance rate of ESBLs producing Klebsiella pneumonia was 100% to ceftazidime, cefalotin, cefotaxime and amoxicillin; but their sensitive rates for meropenem, imipenem, piperacillin/ tazobactam sodium, amikacin and ciprofloxacin were as high as 90.2 % ~100% . Conclusion: The Klebsiella pneumonia that producing ESBLs became a severe resistant strain. We should pay attention on the status of ESBLs distribution and guide the clinical administration of drugs.
