      目的:探讨基质金属蛋白酶-9(matrix metalloproteinase-9,MMP-9)的表达与胰腺癌生物学特性及对患者预后的影响。方法: 采用免疫组织化学法检测63 例原发性胰腺癌及11 例癌旁正常组织中MMP-9 的表达.比较MMP-9 的表达与胰腺癌患者生物学 特性及预后的关系。结果:MMP-9 阳性表达率为68.3%(43/63),而11 例癌旁正常组织无MMP-9 表达。MMP-9 的表达与肿瘤大小 (P=0.014)、分化程度(P=0.039)、TNM 分期(P=0.001)、远处转移(P=0.011)及淋巴结转移(P=0.009)显著相关。MMP-9 表达阳性者生存 时间明显低于阴性表达者(Log-rank 检验,x2=32.70 , P=0.000)。Cox 回归分析表明:分化程度(P=0.000)、TNM 分期(P=0.006)、淋巴 结转移(P=0.035)及MMP-9 过表达(P=0.000)是胰腺癌预后的独立影响因素。结论:MMP-9 过表达在胰腺癌的浸润转移中发挥重 要作用,过表达提示预后不良.对MMP-9 的检测可有效的评估胰腺癌患者的预后。

      Objective: To evaluate the relationship between expression of matrix metalloproteinase-9 (MMP-9) and its biological characteristic, pognosis in pancreatic cancer patients. Methods:The expression of MMP-9 in 63 patients with pancreatic cancer and in 11 patients with the pancreatic tissue of noncarcinoma was examined by immunohistochemical methods. The expression of MMP-9 was compared with biological characteristics and prognosis of pancreatic cancer. Results: The positive expression rate of MMP-9 was 68.3% (43/63), and there was no expression of MMP-9 in adjacent normal pancreatic tissues. The expressions of MMP-9 were significantly correlated with tumor size (P=0.014), differentiation (P=0.039), TNM stages (P=0.001), distant metastasis (P=0.011) and lymph node metastasis (P=0.009). And the survival time of pancreaic cancer patients with the positive expression of MMP-9 was shorter than the negative expression(Log-rank test, x2=32.70,P=0.000). The Cox regression showed that differentiation(P=0.000), TNM stages(P=0.006), lymph node metastasis (P=0.035), MMP-9 overexpression (P=0.000) were related with prognosis of pancreatic cancer patients. Conclusion: The overexpressions of MMP-9 proteins play important roles in proliferation and progression in pancreatic cancer, the overexpression of MMP-9 suggests the worse prognosis in pancreatic cancer. The examination of MMP-9 is useful in evaluating the prognosis of pancreatic cancer patients.
